目的分析乳腺局部结构紊乱的超声表现,探讨高频超声对乳腺局部结构紊乱良恶性病变的诊断价值。方法对82例术前超声诊断为乳腺局部结构紊乱患者的病理资料进行回顾性分析。结果病理结果为40例乳腺腺病,16例纤维腺瘤,15例浸润性癌,4例原位癌,2例肉芽肿性炎,2例导管内乳头状瘤,3例化脓性炎。边缘成角、微钙化、后方衰减及内部回声情况与局部结构紊乱的病理分类有相关性,差异有统计学意义(P<0.05)。ROC曲线分析结果 ,边缘有无成角(灵敏度60.9%,特异度91.5%,曲线下面积0.797)和有无微钙化(灵敏度68.2%,特异度93.3%,曲线下面积0.839)用于诊断局部结构紊乱良恶性有较高效能,且有无微钙化诊断效能最高。结论乳腺局部结构紊乱大部分为腺病等良性病变,但亦有恶变可能;边缘成角和微钙化是诊断乳腺局部结构紊乱良恶性有意义的特征,且有无微钙化诊断效能最高。
OBJECTIVE: To analyze the ultrasonographic features of local structural disorders in breast and to investigate the diagnostic value of high-frequency ultrasound in the diagnosis of benign and malignant lesions with local structural disturbance in breast. Methods A retrospective analysis was performed on the pathological data of 82 patients diagnosed as local structural disorder of breast by ultrasound before operation. Results The pathological results were 40 cases of breast adenosis, 16 cases of fibroadenoma, 15 cases of invasive carcinoma, 4 cases of carcinoma in situ, 2 cases of granulomatous inflammation, 2 cases of intraductal papilloma and 3 cases of purulent inflammation. Edge angulation, micro-calcification, posterior attenuation and internal echo were correlated with pathological classification of local structural disorders, with significant differences (P <0.05). The results of ROC curve analysis showed that there was no angulation at the edges (sensitivity 60.9%, specificity 91.5%, area under the curve 0.797) and presence / absence of microcalcification (sensitivity 68.2%, specificity 93.3%, area under the curve 0.839) Malignant benign and malignant have higher efficacy, and whether the highest diagnostic efficiency of micro-calcification. Conclusions Most of the disorders of breast local structure are adenopathy and other benign lesions, but also have the possibility of malignant transformation. Marginal angulation and micro-calcification are the benign and malignant features in diagnosing the local structural disorders of the breast, with the highest diagnostic efficiency.
