目的观察加巴喷丁复合吗啡预防性镇痛对大鼠慢性压迫坐骨神经(CCI)后导致的机械缩足阈变化以及脊髓背角肿瘤坏死因子-α(TNF-α)表达的影响。方法随机选择重量(180±20)g的成年雄性SD大鼠24只,分为假手术组(S组)、模型组(M组)、预防性镇痛组(P组)和常规镇痛组(N组)。观察不同方式给药的模型组和对照组大鼠的行为学变化。术后10d分离脊髓通过免疫组化检测TNF-α水平的变化。结果术前各组大鼠机械痛阈差异无统计学意义,与M组相比,手术后各时段S、P、N组都存在不同程度的改变,差异均有统计学意义(P<0.05);P、N组TNF-α的积分光密度均值(IOD)水平分别为(0.2185±0.01980)、(0.2301±0.01386),与M组的(0.2902±0.01325)比较明显降低,差异均有统计学意义(P<0.05)。结论加巴喷丁复合吗啡对大鼠采用预防性镇痛有较好的效果,可显著降低CCI后导致的机械痛阈,降低TNF-α在脊髓背角的表达。
Objective To observe the effects of gabapentin combined with morphine on analgesia after chronic constriction injury of sciatic nerve (CCI), and to investigate the effect of prophylactic analgesia with gabapentin on TNF-α in spinal dorsal horn. Methods Twenty-four adult male Sprague-Dawley rats weighing 180 ± 20 g were randomly divided into sham-operation group (S group), model group (M group), prophylactic analgesia group (P group) and conventional analgesia group (N group). To observe the behavioral changes of model rats and control rats in different ways. The spinal cord was separated 10 days after operation to detect the change of TNF-α level by immunohistochemistry. Results There was no significant difference in mechanical pain threshold between groups before operation. Compared with M group, S, P and N groups had different degrees of change at each time after surgery, the differences were statistically significant (P <0.05) ; The integral optical density mean (IOD) levels of TNF-αin P and N groups were (0.2185 ± 0.01980) and (0.2301 ± 0.01386), respectively, which were significantly lower than those in M group (0.2902 ± 0.01325), the differences were statistically significant (P <0.05). Conclusion Gabapentin combined morphine has good effect on preventive analgesia in rats, and can significantly reduce the mechanical pain threshold induced by CCI and decrease the expression of TNF-α in spinal dorsal horn.
