目的探讨针对早发性乳腺癌患者开展彩色多普勒超声检查的临床效果。方法选取本院2013年1月至2016年1月收治的30例早发性乳腺癌患者,均给予彩色多普勒超声检查,以病理检查为基础评估其临床诊断准确率,并总结早发性乳腺癌的影像学特征。结果本组患者的彩色多普勒超声诊断准确率为93.3%(28/30),误诊2例,分别为乳腺炎症性改变与纤维腺瘤。在早发性乳腺癌的超声影像特征中导管内癌和浸润性导管癌彩色多普勒超声表现多为肿块形态不规则,毛刺或呈角边缘,内部呈现低回声且不均匀,肿块后方回声衰减不明显,流信号丰富,以Adler II-III级为主,高速高阻动脉频谱。髓样癌表现为周围高回声晕以及毛刺征,间质内纤维结构改变,腋窝淋巴结异常肿大等。结论彩色多普勒超声对早发性乳腺癌具有良好诊断作用,并且该项检查具备无创、操作简单以及可重复操作等优势,具有确切临床应用价值,有利于筛检早发性乳腺癌。
Objective To investigate the clinical effect of color Doppler ultrasonography in patients with early-onset breast cancer. Methods Thirty patients with early-stage breast cancer admitted from January 2013 to January 2016 in our hospital were examined by color Doppler ultrasonography. The accuracy of clinical diagnosis was evaluated on the basis of pathological examination, Imaging features of breast cancer. Results The diagnostic accuracy of color Doppler ultrasound in this group was 93.3% (28/30) and 2 cases were misdiagnosed as inflammatory changes of breast and fibroadenoma. Intraductal carcinoma and invasive ductal carcinoma in color Doppler ultrasound in the features of ultrasound imaging of early-onset breast cancer are mostly irregular, burr or angular edge, low hypoechoic appearance and uneven internal echo echo attenuation Obviously, the flow of signal-rich, with Adler II-III-based, high-speed high-impedance artery spectrum. Medullary carcinoma showed hyperechoic halo around and burr levy, interstitial changes in fiber structure, abnormal axillary lymph nodes and so on. Conclusion Color Doppler ultrasound has a good diagnostic value for early-stage breast cancer, and this test has the advantages of non-invasive, simple operation and repeatable operation, and has definite clinical value, which is good for screening early-stage breast cancer.
