目的探讨孔源性视网膜脱离(RRD)术后视力恢复情况及其影响因素。方法回顾性分析我院眼科2015年6月~2016年6月期间,以孔源性视网膜脱离为第一诊断并经手术治疗视网膜复位成功的病历资料,共64例64眼。全部采用玻璃体联合视网膜手术。观察RRD患者的发病年龄、视网膜脱离范围、视网膜脱离时间、黄斑是否脱离、手术前后视力、术后裂孔封闭及视网膜复位情况,采用卡方检验对不同年龄、视网膜脱离范围、视网膜脱离时间、黄斑是否脱离的患者术后视力变化情况进行分析,选择差异有统计学意义的因素行Spearman等级相关检验。结果视网膜复位术后视力提高36眼(56.3%),视力不变15眼(23.4%),视力下降13眼(20.3%)。不同黄斑状态、视网膜脱离范围、视网膜脱离时间及年龄的术后视力恢复情况差异有显著性(P<0.05)。对上述因素行Spearman等级相关检验,发现上述术前因素与术后视力关联程度从大到小为:黄斑是否脱离(rs=-0.574,P=0.000)、视网膜脱离范围(rs=-0.498,P=0.000)、视网膜脱离时间(rs=-0.489,P=0.000)、年龄(rs=-0.413,P=0.000)。结论 RRD患者的术后视力恢复与术前黄斑是否脱离、视网膜脱离时间、视网膜脱离范围、年龄有关,其中术前黄斑是否脱离、视网膜脱离范围及时间对术后视力的恢复影响最为显著。
Objective To investigate the visual acuity recovery after rhegmatogenous retinal detachment (RRD) and its influencing factors. Methods A retrospective analysis of our hospital ophthalmology from June 2015 to June 2016, rheumatoid retinal detachment as the first diagnosis and the successful treatment of retinal reattachment medical records, a total of 64 cases 64 eyes. All with vitreoretinal surgery. The incidence of retinal detachment, retinal detachment time, macular detachment, visual acuity before and after surgery, postoperative hilar occlusion and retinal detachment were observed in patients with RRD. Results After retinal reattachment, visual acuity increased by 36 eyes (56.3%), visual acuity unchanged (23.4%), vision decreased by 13 eyes (20.3%). There were significant differences in postoperative visual acuity recovery between different macular state, retinal detachment range, retinal detachment time and age (P <0.05). Spearman rank correlation test showed that the correlation between preoperative factors and postoperative visual acuity was descending (rs = -0.574, P = 0.000), retinal detachment range (rs = -0.498, P = 0.000), retinal detachment time (rs = -0.489, P = 0.000), age (rs = -0.413, P = 0.000). Conclusion The postoperative visual acuity recovery in patients with RRD is closely related to whether preoperative macular detachment, retinal detachment time, extent and age of retinal detachment, and whether the preoperative macular detachment, retinal detachment extent and time have the most significant effect on postoperative visual acuity recovery.
