目的:探讨了七氟烷用于小儿患者时的诱导效果。方法:回顾性分析我院2011年4月~2013年4月收治的患儿80例的临床资料,所用患儿均不给术前药,入手术室后面罩直接吸纯氧(1L/min)加七氟烷,逐步升高七氟烷的吸入浓度(最大浓度为7%),直至患儿睫毛反射消失,维持浓度控制在4.0%以下。所有患儿均行气管插管,可合用维库溴胺0.1mg/kg保证肌松。结果:80例患儿采用七氟烷吸入诱导成功率为100%,意识消失时间在(56.3±10.6)s,睫毛反射消失时间在(63.2±13.4)s之间,插管时间在(168.3±37.6)s;呼吸道分泌物少,肌松良好,插管条件良好,气道耐受性好,无兴奋挣扎现象,麻醉诱导效果满意。流动力学稳定,无心率减慢、血压下降者;全组患儿体温均正常。全组患儿无一例发生呛咳、屏气、喉痉挛、分泌物增加等兴奋期反应,术毕苏醒后呼吸平稳,无喉痉挛、呼吸困难出现,无一例发生胃食管反流误吸现象。结论:七氟烷吸入用于小儿诱导快速,气道刺激小,心血管抑制轻,麻醉满意,安全可控。
Objective: To investigate the induction effect of sevoflurane in pediatric patients. Methods: The clinical data of 80 children admitted to our hospital from April 2011 to April 2013 were retrospectively analyzed. None of the children were given preoperative medication. Results: The success rate of induction of sevoflurane inhalation was 100%, the time of consciousness disappearance was (56.3 ± 10.6) s, the disappearance of eyelash reflex was (63.2 ± 13.4) s and the time of intubation was (168.3 ± 37.6) s; less respiratory secretions, muscle relaxation, good intubation conditions, airway tolerance, no excitement, anesthesia induced satisfactory results. Flow dynamics and stability, no heart rate slowed down, blood pressure were decreased; all children with normal body temperature. None of the children in the group had choking, breath holding, laryngospasm, increased excretion and other excitement reactions. After awakening of the operation, they had stable breathing, no laryngospasm and dyspnea. None of the patients had gastroesophageal reflux aspiration. Conclusion: Sevoflurane inhalation for rapid induction of children, small airway stimulation, cardiovascular inhibition, satisfactory anesthesia, safe and controllable.
