听觉统合训练对整体发育迟缓儿童语言及情绪-社会性的影响

目的:探讨听觉统合训练(auditory integration training,AIT)对整体发育迟缓(global developmental delay,GDD)儿童语言及情绪-社会性的治疗效果。方法:144例GDD儿童随机分成治疗组、对照组和空白对照组,每组轻度GDD 28例,中重度GDD 20例。运用脑干诱发电位仪行ASSR测试判定听觉敏感频率,数码听觉统合训练仪进行AIT治疗,采用0—6岁小儿神经心理发育检查表、AIT疗效调查表,治疗前后3个月语言发育商、总发育商评分差异及AIT治疗3个月后临床症状的变化来评估整体疗效。结果:治疗3个月后,治疗组中轻度GDD语言DQ、总DQ较治疗前明显提高(P<0.001),而治疗组中中重度GDD患儿以及全部对照组中患儿语言DQ、总DQ较治疗前提高不明显,差异无显著性意义(P>0.05),全部空白对照组语言DQ、总DQ较治疗前明显降低(P<0.01,P<0.05)。轻度GDD三组患儿治疗后3个月语言DQ、总DQ差异有非常显著性意义(P<0.001),中重度GDD三组患儿治疗后3个月总DQ差异有显著性意义(P<0.05)。治疗后两两比较显示:轻度GDD治疗组与空白对照组语言DQ、总DQ比较差异显著(P<0.01,P<0.01),中重度GDD治疗组与空白对照组总DQ比较差异显著(P<0.05),轻度、中重度GDD对照组与空白对照组总DQ比较差异显著(P<0.05),轻度GDD对照组与空白对照组语言DQ比较差异无显著性意义(P>0.05)。AIT治疗3个月后在语言、社会交往上平均总有效率60.42%、72.92%,控制情绪能力平均总有效率20.83%。结论:AIT能促进轻度GDD儿童语言发育水平,提高GDD儿童社会交往能力及改善情绪。 Objective: To investigate the therapeutic effect of auditory integration training (AIT) on language and emotion-sociality in children with global developmental delay (GDD). Methods: 144 GDD children were randomly divided into treatment group, control group and blank control group. Each group had 28 mild GDDs and 20 moderate and severe GDDs. ASSR test was used to determine the frequency of auditory sensory acuity using Brainstem Evoked Potentials. AIT was performed on the Digital Auditory Trainer. The neuropsychological development checklist of children 0-6 years old, AIT efficacy questionnaire, language development providers 3 months before and after treatment Developmental score differences and AIT treatment 3 months after the clinical symptoms to assess the overall effect. Results: After 3 months of treatment, the mild GDD language DQ and total DQ in the treatment group were significantly higher than those before treatment (P <0.001). However, in the treatment group, there was no significant difference in language DQ between the children with moderate and severe GDD and the control group DQ was not significantly higher than that before treatment (P> 0.05). The DQ and total DQ in all blank control groups were significantly lower than those before treatment (P <0.01, P <0.05). There was a significant difference in language DQ and total DQ between the three groups at 3 months after treatment (P <0.001), and there was significant difference in total DQ between the 3 groups <0.05). After treatment, the comparisons between two groups showed that there was significant difference in language DQ and total DQ between the mild GDD treatment group and the blank control group (P <0.01, P <0.01), and there was significant difference in the total DQ between the moderate and severe GDD treatment group and the blank control group <0.05). There was significant difference in total DQ between mild and moderate GDD control group and blank control group (P <0.05), but there was no significant difference between mild GDD control group and blank control group (P> 0.05). After 3 months of AIT treatment, the average total effective rate in language and social interaction was 60.42% and 72.92%, and the average total effective rate in controlling emotional ability was 20.83%. Conclusion: AIT can promote the language development of GDD children with mild GDD, improve the social interaction ability and improve the mood of GDD children.
