目的 了解新鲜羊膜移植在早期角膜碱烧伤治疗中抑制炎症和促上皮愈合的作用 ,并与保存羊膜作比较。方法 以 48只新西兰大白兔制作角膜碱烧伤模型 ,2 4h后设立新鲜羊膜移植组、保存羊膜移植组和对照组。术后 14、30 d取材角膜进行组织病理学检查 ,以计算机图像分析系统测量其多形核白细胞含量 ,扫描电镜和透射电镜观察角膜超微结构改变。结果 新鲜羊膜移植组角膜组织中多形核白细胞含量低于保存羊膜移植组 ,二者均低于对照组 (P<0 .0 1)。组织病理及超微结构观察显示 ,在角膜上皮细胞排列、细胞表面及细胞内结构方面 ,新鲜羊膜移植组优于保存羊膜移植组 ,二者均较对照组好。结论 在角膜碱烧伤急性期治疗中新鲜羊膜移植较保存羊膜移植具有较强的抑制炎症及促上皮愈合作用
Objective To investigate the effect of fresh amniotic membrane transplantation on the inhibition of inflammation and epithelial healing in the treatment of early corneal alkali burn and to compare with the preservation of amniotic membrane. Methods The corneal alkali burn model was made in 48 New Zealand white rabbits. Fresh amniotic membrane transplantation group was established after 24 hours, and the amniotic membrane transplantation group and the control group were preserved. The corneas were harvested at 14 and 30 days after operation for histopathological examination. The contents of polymorphonuclear leukocytes were measured by computer image analysis system. The ultrastructural changes of cornea were observed by scanning electron microscopy and transmission electron microscopy. Results The content of polymorphonuclear leukocytes in fresh amniotic membrane transplantation group was lower than that in preserved amniotic membrane transplantation group, both of which were lower than that of control group (P <0.01). Histopathological and ultrastructural observation showed that the fresh amniotic membrane transplantation group was better than the preserved amniotic membrane transplantation group in corneal epithelial cell arrangement, cell surface and intracellular structure, both of which were better than the control group. Conclusion Fresh amniotic membrane transplantation in the acute phase of corneal alkali burn has stronger inhibition of inflammation and promoting epithelial healing compared with preserved amniotic membrane transplantation
