目的:探讨急性冠状动脉综合征(ACS)患者冠状动脉Gensini评分与血清超敏C反应蛋白(hsCRP)水平的相关性。方法:回顾性分析2013-08-2014-08我院内科治疗的135例ACS患者临床资料,依据Gensini评分判定冠状动脉病变的严重程度,并将其分为3组,重度病变组46例,中度病变组45例,轻度病变组44例,另选择冠状动脉造影(CAG)无明显异常者45例作为对照组,观察4组冠状动脉狭窄程度,并对比4组Gensini评分,以Logistic回归模型、Spearman相关性分析判定各危险因素与Gensini评分的关系。结果:与对照组比较,ACS各组血清hs-CRP水平均明显升高,差异有统计学意义(P<0.05),以Gensini评分为因变量,以年龄、SBP、hs-CRP、低密度脂蛋白(LDL-C)、空腹血糖(FPG)为自变量,实施多元Logistic回归分析,提示年龄、SBP、hs-CRP、LDL-C、FPG是冠状病变狭窄程度的独立危险因素。实施Spearman相关性分析,将Gensini评分作为变量,判定Gensini评分与hs-CRP水平的相关性,提示hs-CRP水平与Gensini评分呈正相关(P<0.01,r=0.915)。结论:ACS患者通过检测血清hs-CRP水平,能够对冠状动脉狭窄程度进行预测,方便快速,可以为临床提供诊断依据。
Objective: To investigate the relationship between coronary artery Gensini score and serum hsCRP level in patients with acute coronary syndrome (ACS). Methods: The clinical data of 135 patients with ACS treated in our hospital from 2013-08-2014-08 were retrospectively analyzed. The severity of coronary artery disease was determined according to Gensini score. The patients were divided into 3 groups, 46 patients with severe lesion, 45 cases of degree lesion group and 44 cases of mild lesion group. Another 45 cases without obvious abnormalities of coronary artery angiography (CAG) were selected as the control group. The degree of coronary artery stenosis was observed in 4 groups. The Gensini score was compared with 4 groups. Logistic regression model , Spearman correlation analysis to determine the relationship between risk factors and Gensini score. Results: Compared with the control group, serum hs-CRP levels in all ACS groups were significantly increased (P <0.05). The Gensini score was used as the dependent variable to measure the serum levels of hs-CRP, age, SBP, LDL-C and FPG were independent variables. Multivariate Logistic regression analysis showed that age, SBP, hs-CRP, LDL-C and FPG were independent risk factors for coronary artery stenosis. Spearman’s correlation analysis and Gensini’s score were used as variables to determine the correlation between Gensini score and hs-CRP level, which suggested that there was a positive correlation between hs-CRP level and Gensini score (P <0.01, r = 0.915). Conclusion: The serum hs-CRP level in ACS patients can predict the degree of coronary artery stenosis, which is convenient and rapid. It can provide the basis for clinical diagnosis.
