目的探讨餐前短效胰岛素与甘精胰岛素联合胰岛素泵短期强化治疗2型糖尿病的临床疗效。方法选取云南省大理州人民医院2014年1月—2016年1月收治的2型糖尿病患者100例,按照随机数字表法将患者分为对照组与观察组,各50例。对照组患者予以胰岛素泵短期强化治疗,观察组患者在对照组治疗基础上予以甘精胰岛素+短效胰岛素治疗,两组患者均连续治疗3个月。比较两组患者治疗前后血糖(空腹血糖、餐后2 h血糖)水平、胰岛β细胞功能[空腹胰岛素(FINS)、胰岛素抵抗指数(HOMA-IR)、胰岛素分泌指数(HOMA-β)]。结果治疗前两组患者空腹血糖、餐后2 h血糖水平比较,差异无统计学意义(P>0.05),治疗后观察组患者空腹血糖、餐后2 h血糖水平低于对照组(P<0.05)。治疗前两组患者FINS、HOMA-IR、HOMA-β比较,差异无统计学意义(P>0.05);治疗后观察组患者FINS、HOMA-IR高于对照组,HOMA-β低于对照组(P<0.05)。结论餐前短效胰岛素与甘精胰岛素联合胰岛素泵短期强化治疗2型糖尿病的临床疗效确切,可有改善患者血糖水平及胰岛β细胞功能。
Objective To investigate the clinical efficacy of short-acting pre-prandial insulin and insulin-glargine plus insulin pump in short-term intensive treatment of type 2 diabetes mellitus. Methods A total of 100 type 2 diabetic patients admitted from January 2014 to January 2016 in Dali People’s Hospital of Yunnan Province were selected and divided into control group and observation group according to random number table. Patients in the control group were treated with short-term intensive insulin pump. Patients in the observation group were treated with insulin glargine and short-acting insulin on the basis of the control group. Patients in both groups were treated continuously for 3 months. The levels of blood glucose (fasting blood glucose, postprandial blood glucose 2 h), islet β-cell function [fasting insulin (FINS), insulin resistance index (HOMA-β), insulin secretion index (HOMA-β)] were compared between the two groups before and after treatment. Results There was no significant difference in fasting blood glucose and postprandial 2h blood glucose levels between the two groups before treatment (P> 0.05). After treatment, fasting blood glucose and postprandial 2h blood glucose levels in the observation group were lower than those in the control group (P <0.05) ). There was no significant difference in FINS, HOMA-IR and HOMA-β between the two groups before treatment (P> 0.05). FINS and HOMA-IR in the observation group were higher than those in the control group P <0.05). Conclusion Short-term pre-meal insulin and insulin glargine insulin pump short-term intensive treatment of type 2 diabetes clinical efficacy is exact, can improve blood glucose levels and pancreatic β-cell function.
