近年来通过对房室结的研究发现,房室结的致密结产生了三个后延伸:一个在冠状窦方向沿着三尖瓣环的右下后延伸(假定的慢径),一个在Koch三角的前上方紧邻致密结的过渡组织区(假定的快径),另一个在二尖瓣环方向(左房后延伸).三个后延伸形成了房室结双径路多种电生理特性的解剖学基础及手术消融的解剖结构,为房室结的双径路或多径路形成、房室结折返性心动过速折返环的部位、房室结折返性心动过速的分类及导管消融策略的新观点奠定基础.“,”In recent years,the study of atrioventricular(AV)node found that the compact AV node gave rise to three posterior extensions:one in the direction of the coronary sinus along the tricuspid annulus(the putative“slow pathway”),a second in the anterior portion of the triangle of Koch near the compact portion of the AV node(the putative“fast pathway”),and the third in the direction of the mitral annulus(the left atrial extension).Three posterior extensions formed anatomical basis of various electrophysiological properties of dual atrioventricular nodal pathways and anatomical structures of ablation.Research studies have shown that the posterior extension of the AV node provides the anatomic basis for all electrophysiologic properties and form the basis for recognition about the mechanisms of dual or multiple AV nodal pathways,the classification of AVNRT and catheter ablation of AVNRT.
