近年来,随着肝脏外科的发展,肝细胞癌做肝切除的成功率有所增加.尽管肝癌长期存活已有散在报道.但是预后普遍较悲观.而且,肝细胞癌常伴发于肝硬化,从而限制了肝切除手术的进行,影响了长期存活。本文报道2例伴发于肝硬化的肝细胞癌病人,在肝切除后,已生存5年以上.一例伴发于血吸虫肝硬化,肝右叶切除后,已存活5年零2个半月,后死于肝功能衰竭.尸检发现,在残留肝叶中有2个直径各为1 cm 的复发肿瘤,但并非是致死的原因。另一病人,在扩大的肝右叶切除后,9年2个月仍然存活。此例肿瘤体积较大,切除的标本重达2,800克。
In recent years, with the development of hepatic surgery, the success rate of hepatectomy for liver cancer has increased. Although long-term survival of liver cancer has been scattered, but the prognosis is generally more pessimistic. Moreover, hepatocellular carcinoma is often associated with cirrhosis. This limits the progress of liver resection and affects long-term survival. This article reports 2 cases of hepatocellular carcinoma patients with cirrhosis who survived for more than 5 years after hepatectomy. One case was associated with schistosomiasis cirrhosis. After right hepatectomy, he had survived for 5 years and 2 months and a half months. He died of liver failure. On post mortem examination, there were two recurring tumors with a diameter of 1 cm in the left hepatic lobe, but this was not the cause of death. Another patient survived 9 months and 2 months after enlarging the right hepatectomy. In this case, the tumor was larger and the resected specimen weighed 2,800 grams.
