目的通过比较经颅多普勒(TCD)与数字减影血管造影(DSA)在大脑中动脉狭窄中的诊断价值,为临床提供方便可靠的检查方法。方法对经TCD检查确诊为大脑中动脉狭窄的60例患者行DSA检查,对2种检查结果进行比较分析。结果经TCD确诊为大脑中动脉狭窄的病例,病变侧狭窄处的血流速率均明显高于健侧1倍以上,在动脉粥样硬化性狭窄中多伴有“海鸥鸣”或“铁锅炒沙”样杂音,且频谱形态呈圆钝或搏动指数值增高的阻力波形。该60例在DSA检查中均发现不同部位的不同狭窄。结论TCD检查具有快速、方便、安全、经济和确诊率高的优点,可作为脑血管病的首选检查手段。
Objective To compare the diagnostic value of transcranial Doppler (TCD) and digital subtraction angiography (DSA) in the diagnosis of middle cerebral artery stenosis and provide a convenient and reliable method for clinical examination. Methods Sixty patients diagnosed as middle cerebral artery stenosis by TCD were examined by DSA, and the results of two examinations were compared. Results TCD was diagnosed as middle cerebral artery stenosis, the lesion side of the stenosis at the blood flow rate was significantly higher than 1 times more than in the atherosclerotic stenosis often accompanied by “gull Ming” or “wok fried Sand ”like noise, and the shape of the spectrum is blunt or pulsatility index value increased resistance waveform. The 60 patients in the DSA examination found in different parts of different stenosis. Conclusion TCD examination has the advantages of fast, convenient, safe, economical and high diagnosis rate, which can be used as the first choice of examination for cerebrovascular disease.
