目的评估在下颌低位近中阻生智齿拔除时其远中去骨或增隙的作用,并比较和分析高速涡轮牙钻与骨凿在拔除下颌低位近中阻生智齿中的优点。方法 2006年8月至2011年5月诊治下颌阻生智齿241例,随机分为2组,治疗组153例采取高速涡轮牙钻切割牙冠,去除颊侧及远中部分骨质,行智齿远中增隙拔除;对照组88例采取骨凿劈冠、去骨拔除。观察临床效果,及时记录拔牙时间、术后肿胀程度、张口受限及术中并发症的发生情况。结果治疗组手术创伤小,手术时间短,术中并发症较少,术后肿胀、疼痛和张口受限程度明显低于对照组(P<0.05)。结论涡轮钻拔除下颌低位近中阻生智齿具有时间短、创伤小和并发症少等优点。远中去骨或增隙可以减小下颌低位近中阻生智齿的拔除阻力,减少拔牙时间和术后不良反应。
Objective To evaluate the role of distal bone removal or crevice in the removal of distal mandibular wisdom teeth, and to compare and analyze the advantages of high-speed turbine dental drill and osteotome in the removal of mandibular lower probiotics. Methods From August 2006 to May 2011, 241 patients with impacted mandibular wisdom teeth were randomly divided into two groups. 153 cases of the treatment group were given high-speed turbine dental drill to cut the crowns, remove the buccal and distal part of the bone, In the gap removed; control group of 88 cases of bone chisel split crown, bone removal. Observe the clinical effect, timely record extraction time, postoperative swelling, limited mouth opening and intraoperative complications. Results The treatment group had less operation trauma, shorter operation time, fewer intraoperative complications, and less postoperative swelling, pain and mouth opening restriction than the control group (P <0.05). Conclusion Turbine drill removal in addition to mandibular low near probing wisdom teeth with short time, less trauma and fewer complications. Distal bone or crevice can reduce the mandibular lower extremity induced wisdom tooth removal resistance, reduce the extraction time and postoperative adverse reactions.
