目的:探讨胃癌病人手术前后外周血中微转移标志物癌胚抗原(CEA)mRNA表达及临床意义。方法:采用巢式实时PCR技术检测50例胃癌病人手术前后的外周血中CEA mRNA的表达情况,并选择20名健康志愿者作为阴性对照组,选择10例新鲜胃癌组织作为阳性对照组。结果:胃癌病人术前外周血中CEA mRNA的表达阳性率为48.00%(24/50),术后阳性率为60.0%(30/50),差异无统计学意义(P>0.05);其阳性表达率与肿瘤的组织学类型及临床分期间具有相关性(P<0.05),而与病人性别、年龄及有无淋巴结转移间无统计学相关性(P>0.05);20名健康志愿者外周血中均无CEA mRNA阳性表达;而在10例新鲜胃癌组织中,均显示阳性表达。结论:CEA mRNA的表达可能与肿瘤微转移相关,采用巢式RT-PCR方法检测外周血中CEA mRNA的表达,可能是一种较理想的预测方法。手术对微转移影响尽管存在,但在临床上尚无统计学意义。
Objective: To investigate the expression of CEA mRNA in peripheral blood of gastric cancer patients before and after surgery and its clinical significance. Methods: The expression of CEA mRNA in peripheral blood of 50 patients with gastric cancer before and after operation was detected by nested real-time PCR. Twenty healthy volunteers were selected as negative control group, and 10 fresh gastric cancer tissues were selected as positive control group. Results: The positive rate of CEA mRNA in preoperative peripheral blood of patients with gastric cancer was 48.00% (24/50) and the positive rate was 60.0% (30/50), the difference was not statistically significant (P> 0.05). The positive rate There was no significant correlation between the expression rate and the histological type and clinical stage (P <0.05), and with the gender, age and lymph node metastasis (P> 0.05). The peripheral of the 20 healthy volunteers CEA mRNA was not expressed in the blood, while in 10 cases of fresh gastric cancer tissues, all of them showed positive expression. Conclusion: CEA mRNA expression may be related to tumor micrometastasis. Using nested RT-PCR to detect the expression of CEA mRNA in peripheral blood may be an ideal predictor. The effect of surgery on micrometastasis, although present, is not clinically significant.
