为研究急性髓系白血病 (AML)细胞 P170表达与白血病细胞体外集落生长之间的关系及其预后意义 ,对 31例 AML 住院患者骨髓细胞用 Dicke法进行 AML 白血病集落 (AML- CFU)体外培养 ,并用单抗免疫组化法对骨髓细胞和 AML- CFU细胞 P170表达进行检测。 2 8例 AML 患者采用蒽环类药 + Ara- c治疗 ,2个疗程后评定疗效。结果显示 :初发、复发 AML、慢粒急粒变及 CR期 AML 患者 AML- CFU集落分别为 172 .9± 72 .1/ 2×10 5 ,176 .0± 76 .6 / 2× 10 5 ,2 2 4.5± 5 4.7/ 2× 10 5和 9.7± 5 .0 / 2× 10 5 ,初、复发 AML 及 CML 急粒变患者 AML-CFU明显高于 CR期 AML 患者 (P<0 .0 1) ,而初、复发 AML 及慢粒急粒变患者 AML- CFU无差异。 AML 患者AML- CFU细胞和 BM细胞 P170阳性表达和 P170阴性表达均为 16例。两种细胞 P170阳性表达一致者 13例 ,P170阴性表达一致者 12例 ,P170阳性和 P170阴性表达不一致者各 3例 (P=0 .114)。 P170阴性者 CR10例 (10 /13,76 .9% ) ,P170阳性者 11例 CR2例 (2 / 11,18.2 % ) ,P=0 .0 0 48。多元回归及逐步回归分析 AML 患者 CR率与P170表达及疾病状态有关 ,其相关系数分别为 - 0 .9786和 - 0 .40 5 5 (P值分别为 0 .0 0 94和 0 .0 10 5 )。以上可见 ,AML- CFU数量与疾病活跃
In order to study the relationship between the expression of P170 in acute myeloid leukemia (AML) cells and the colony growth of leukemic cells in vitro and its prognostic significance, 31 cases of AML inpatient bone marrow cells were cultured with Acute myeloid leukemia (AML-CFU) The expression of P170 in bone marrow cells and AML-CFU cells was detected by immunohistochemistry. Twenty-eight patients with AML were treated with anthracycline plus Ara-c and evaluated after 2 courses of treatment. The results showed that the AML-CFU colonies in the primary, recurrent AML, CML and CR AML patients were 172.9 ± 72.1 / 2 × 10 5, 176.0 ± 76.6 / 2 × 10 5 , 22.5 ± 5 4.7 / 2 × 10 5 and 9.7 ± 5 .0 / 2 × 10 5 respectively. The AML-CFU in primary and recurrent AML and CML patients was significantly higher than that in CR patients with AML (P0.01 1), while there was no difference in AML-CFU between the first and the recurrent AML and the CML patients. AML patients with AML-CFU cells and BM cells P170 positive expression and P170 negative expression were 16 cases. Thirteen cases were consistent with the positive expression of P170 in both cells, 12 cases were consistent with P170 negative expression, and 3 cases were inconsistent with P170 positive expression and P170 negative expression (P = 0.114). In P170-negative patients, CR10 cases (10/13, 76.9%) showed CR2 in 11 cases of P170-positive patients (2/11, 18.2%), P = 0. 0488. Multiple regression and stepwise regression analysis showed that the CR rate in patients with AML was related to the expression of P170 and the disease status, and the correlation coefficients were -0.9786 and -0.054.5 (P = 0.094 and 0.0105 respectively ).
