目的探讨乳腺癌病理组织分级与MR DWI的表观扩散系数(ADC)值是有否相关性。方法前瞻性分析2008年8月至2012年7月间临床拟诊为乳腺癌女患者34例,均行乳腺MRI及手术病理检查,符合入组者20例,年龄32~83岁,中位年龄43.5岁。测量肿块ADC值(避免肿块的液化坏死区),并与对侧乳腺的相应正常区域的ADC值作对照。根据ADC值与乳腺癌病理组织分级列表,采用不同组间均数的两两比较LSD-t检验行相关分析。结果正常乳腺与不同病理分级乳腺癌ADC值多组间两两比较,正常乳腺组织ADC值为(1.89±0.061)×10-3mm2/s,乳腺癌病理组织Ⅰ级ADC值为(1.09±0.122)×10-3mm2/s,乳腺癌病理组织Ⅱ级ADC值为(0.579±0.067)×10-3mm2/s,乳腺癌病理组织Ⅲ级ADC值为(0.203±0.648)×10-3mm2/s各组间差异有统计学意义(P值均<0.001)。乳腺癌病理组织学分级与其ADC值呈负相关(r=-0.969,P=0.000)。结论乳腺癌ADC值与乳腺癌病理组织分级有相关性。因此,测量乳腺癌的ADC值可以预测乳腺癌的恶性程度。
Objective To investigate the relationship between histopathological grade of breast cancer and apparent diffusion coefficient (ADC) of MR DWI. Methods A prospective analysis of 34 women with clinically diagnosed breast cancer from August 2008 to July 2012 were performed breast MRI and surgical pathology. All patients were enrolled in this study. Twenty patients, aged 32-83 years and median age 43.5 years old. The ADC value of the lumps (liquefied necrotic area to avoid the lumps) was measured and compared to the ADC value of the corresponding normal area of the contralateral breast. According to the ADC value and histopathological grade of breast cancer, LSD-t test was used to analyze the correlation between different groups. Results The ADC value of normal breast tissue was (1.89 ± 0.061) × 10-3mm2 / s and that of grade Ⅰ breast cancer tissue was (1.09 ± 0.122) × 10-3mm2 / s, the grade Ⅱ ADC value of breast cancer pathological tissue was (0.579 ± 0.067) × 10-3mm2 / s, the grade Ⅲ ADC value of breast cancer pathological tissue was (0.203 ± 0.648) × 10-3mm2 / s The difference was statistically significant (P <0.001). The histopathological grade of breast cancer was negatively correlated with the ADC value (r = -0.969, P = 0.000). Conclusion The ADC value of breast cancer is correlated with the pathological grade of breast cancer. Therefore, measuring the ADC value of breast cancer can predict the malignancy of breast cancer.
