目的探讨食管癌根治术后主要并发症的原因及防治措施。方法分析200例食管癌根治术的临床资料,总结其主要的并发症发生原因,总结经验教训,探讨预治措施。结果食管癌根治术后主要并发症发生率为18%(36例),其中肺部并发症14%(28例),吻合口瘘2.0%(4例),乳糜胸1.0%(2例),吻合口狭窄0.5%(1例),脓胸0.5%(1例)。结论食管癌根治术后并发症肺部并发症最多见,其次为吻合口瘘,二者均为比较严重的并发症。做好充分术前准备,治疗术前合并疾病,判断患者术前个体情况,严格规范手术操作,术后加强管理,术后足量抗感染及充分的支持疗法,是预防并发症的主要措施。
Objective To investigate the causes and prevention measures of major complications after radical operation of esophageal cancer. Methods The clinical data of 200 cases of esophageal cancer radical mastectomy were analyzed, the main causes of the complications were summarized, the experiences and lessons were summarized, and the pre-treatment measures were discussed. Results The incidence of major complications after radical resection of esophageal cancer was 18% (36 cases), of which pulmonary complications were 14% (28 cases), anastomotic fistula 2.0% (4 cases), chylothorax 1.0% (2 cases) Anastomotic stricture 0.5% (1 case), empyema 0.5% (1 case). Conclusions Complications after radical resection of esophageal cancer are the most common complications of the lungs, followed by anastomotic fistula, both of which are more serious complication. Preoperative preparation, treatment of preoperative preoperative disease, to determine the patient’s preoperative individual situation, and strictly regulate the operation, postoperative management, adequate postoperative anti-infection and adequate supportive therapy is the main measure to prevent complications.
