To the editor:Left ventricular (LV) free wall rupture (FWR) is the most fearful complication after acute myocardial infarction (AMI).We present a rare case that the patient underwent successful late reperfusion but suffered catastrophic irreversible collapse during the procedures.rnA 71-year-old ST elevation myocardial infarction (STEMI) man complained of typical chest pain for 24 hours,with a history of hypertension and smoking,no dyslipidemia,or diabetes mellitus.On admission,ECG showed significant mild ST-segment elevation (leads Ⅱ,Ⅲ,aVF,V5-9) and inverted T waves (leads Ⅰ,Ⅱ,Ⅲ,aVL,aVF,V5-9).Medical treatment with aspirin,clopidogrel,atorvastatin and benapril,low molecular weight heparin (LMWH),and no beta blocker because of sinus bradycardia for six days,coronary angiography showed 70% of middle left anterior descending (LAD),50% of distal LAD,80% of the large first diagonal branch;diffuse lesions of large left circumflex coronary artery (LCX),80%-90%of middle LCX with thrombus signs;60% to 80% stenoses of right coronary artery (RCA).The LCX was identified as culprit lesion.
