目的探讨超声引导下穿刺活检对乳腺肿块的诊断价值。方法选取辽宁省丹东市第一医院2013年6月至2016年6月收治的63例(65例肿块)乳腺疾病患者资料进行回顾性分析,对超声发现的65例乳腺肿块采用美国生产的Bard自动活检枪及16G活检针,在高频超声引导下经皮穿刺活检取得病理结果,并与手术后病理诊断进行对照。结果超声引导下穿刺病理结果检出39例为恶性,其中乳腺浸润性导管癌25例(38.5%),淋巴瘤14例(21.5%);26例为良性,其中炎症疾病2例(3.1%),纤维腺瘤9例(13.8%),良性叶状肿瘤15例(23.1%);64例与手术后病理结果相符,出现1例误诊,将乳腺浸润性导管癌诊断为良性叶状肿瘤,与手术后病理结果差异无统计学意义(P>0.05)。超声引导下穿刺活检对乳腺肿块诊断的准确率为98.5%,灵敏性为97.5%,特异性为100.0%,阳性预测率为100.0%,阴性预测率为96.2%。结论超声引导下穿刺活检对乳腺肿块的诊断微创、安全、准确,可进一步用于指导外科手术及内科治疗。
Objective To investigate the value of ultrasound-guided biopsy in the diagnosis of breast masses. Methods The data of 63 cases (65 cases) of breast diseases admitted to the First Hospital of Dandong City in Liaoning Province from June 2013 to June 2016 were retrospectively analyzed. The 65 cases of breast masses detected by ultrasound were detected by Bard automatic Biopsy gun and 16G biopsy needle, pathological results were obtained by percutaneous biopsy under the guidance of high-frequency ultrasound and compared with the postoperative pathological diagnosis. Results Thirty-nine cases were malignant, including 25 cases (38.5%) with invasive ductal carcinoma and 14 cases (21.5%) with lymphoma. 26 cases were benign, including 2 inflammatory diseases (3.1%), , Fibroadenoma in 9 cases (13.8%) and benign phyllodes tumor in 15 cases (23.1%). Sixty-four cases were consistent with the postoperative pathological findings and one case was misdiagnosed. The diagnosis of breast invasive ductal carcinoma was benign phyllodes tumor, Pathological results after surgery showed no significant difference (P> 0.05). The diagnostic accuracy of ultrasound - guided biopsy on breast masses was 98.5%, sensitivity was 97.5%, specificity was 100.0%, positive predictive value was 100.0% and negative predictive value was 96.2%. Conclusion Ultrasound - guided biopsy is a minimally invasive, safe and accurate diagnosis of breast masses. It can be used to guide the surgical and medical treatment.
