目的:分析心电图在肺栓塞部位与溶栓治疗前后的特点。方法:选取2012年1月-2015年1月收入医院治疗的60例肺栓塞患者为研究对象,对其临床资料进行回顾性分析,观察不同梗塞部位与溶栓前后患者心电图的变化情况。结果:上叶栓塞的心电图变化程度较下叶弱,单节段栓塞的心电图变化程度较多节段弱。左肺栓塞的心电图特点为S1,TⅢ,TV1-TV2,TV1-TV5倒置;而右肺栓塞的心电图特点为S1,S1QⅢTⅢ,QⅢTⅢ,TⅢ,TV1-TV2倒置,SV1~SV5R段粗顿、挫折。溶栓治疗后患者心率减慢,电轴减小,与治疗前比较,差异具有统计学意义(P<0.05);46例患者溶栓前心电图以S1加深,S1QⅢTⅢ、QⅢTⅢ以及TⅢ倒置最为多见;溶栓治疗后大部分心电图的ST-T段改善,RavR下降,S1变浅,TⅢ倒置改善,Q波减小,TV1-TV2倒置加深。结论:肺栓塞部位的不同,心电图检查结果也不同,可通过心电图变化情况了解肺栓塞部位。溶栓治疗后,采用心电图检查有助于对患者病情改善程度的了解。
Objective: To analyze the characteristics of electrocardiogram before and after pulmonary embolism and thrombolytic therapy. Methods: Sixty patients with pulmonary embolism who received hospital admission from January 2012 to January 2015 were enrolled in this study. The clinical data of the patients were retrospectively analyzed. The changes of electrocardiogram in different infarction areas and before and after thrombolysis were observed. Results: The changes of electrocardiogram of upper embolism were weaker than that of lower lobe, and the changes of electrocardiogram of single embolic embolization were more fragile. Left pulmonary embolism ECG characteristics of S1, TIII, TV1-TV2, TV1-TV5 inverted; and right pulmonary embolism ECG characteristics of S1, S1QIIITⅢ, QIIITⅢ, TⅢ, TV1-TV2 inversion, SV1 ~ SV5R section of Dayton, frustration. Thrombolytic therapy in patients with heart rate slowed down, the axis decreased, compared with before treatment, the difference was statistically significant (P <0.05); 46 patients before thrombolysis with S1 deepening, S1QIIITⅢ, QIIITⅢ and T Ⅲ inversion most common After thrombolysis, ST-T of most electrocardiogram improved, RavR decreased, S1 decreased, TⅢ inverted improved, Q wave decreased and TV1-TV2 inverted deepened. Conclusion: Different pulmonary embolism location, ECG test results are also different, through the ECG changes to understand the pulmonary embolism site. After thrombolytic therapy, the use of ECG can help to improve the understanding of the patient’s condition.
