目的探讨川崎病(KD)患儿血浆脑利钠肽(BNP)水平变化及其与心功能的相关性。方法选取KD患儿42例作为KD组,上呼吸道感染患儿30例作为对照组。采用酶联免疫法测定2组患儿的白细胞(WBC)、C反应蛋白(CRP)、红细胞沉降率(ESR)、血浆心肌肌钙蛋白(c Tn I)及血浆BNP。并采用多普勒超声心动图仪分别于急性期和恢复期获取KD患儿的心脏指数(CI)、舒张期二尖瓣血流频谱速度(E/A)、左室短轴缩短率(LVSF)和左室射血分数(LVEF)等参数。结果急性期KD患儿WBC、CRP、ESR、c Tn I及BNP明显高于对照组(P<0.05),WBC、CRP及BNP明显高于恢复期(P<0.05);而CI、LVSF及LVEF明显低于恢复期(P<0.05);血浆BNP与其CI、LVSF及LVEF均呈负相关(r值=-0.602、-0.564、-0.547、P<0.05)。结论急性期KD患儿血浆BNP会异常上升,恢复期逐渐下降,且患儿血浆BNP水平与心功能障碍存在正相关,检测血浆BNP水平对KD的及早诊断有指导意义。
Objective To investigate the changes of plasma brain natriuretic peptide (BNP) in children with Kawasaki disease (KD) and its correlation with cardiac function. Methods 42 children with KD were selected as KD group and 30 children with upper respiratory tract infection as control group. The WBC, C-reactive protein (CRP), erythrocyte sedimentation rate (ESR), plasma cTnI and plasma BNP in two groups were detected by enzyme-linked immunosorbent assay. The indexes of cardiac index (CI), diastolic mitral flow velocity (E / A), left ventricular fractional shortening (LVSF ) And left ventricular ejection fraction (LVEF) and other parameters. Results The WBC, CRP, ESR, cTn I and BNP in children with acute KD were significantly higher than those in control group (P <0.05), while the WBC, CRP and BNP were significantly higher than those in recovery (P <0.05) (P <0.05). Plasma BNP was negatively correlated with CI, LVSF and LVEF (r = -0.602, -0.564, -0.547, P <0.05). Conclusions Acute phase of KD children with abnormal plasma BNP will rise, the recovery period decreased, and children with plasma BNP levels and cardiac dysfunction there is a positive correlation, the detection of plasma BNP levels for early diagnosis of KD is instructive.
