目的 :探讨唾液酸 ( SA)、多胺 ( PA)、癌胚抗原 ( CEA)和糖蛋白抗原 2 4 2 ( CA2 4 2 )等肿瘤标记物 ( TM)单项和多项联合检测在大肠癌定性诊断中的价值。 方法 :应用 HP% M高效液相色谱仪和HP1 0 4 0 A荧光检测器或酶联免疫吸附法测定 70例大肠癌和 68例健康成人血清中的 SA、PA、CEA、CA2 4 2水平 ,分别计算单项和多项 TM联合检测对大肠癌的敏感性、特异性、阳性预测率、阴性预测率和总有效率。结果 :4种 TM在大肠癌病人中均较健康成人显著升高 ,单项指标以 CA2 4 2的诊断价值最高 ,其次是 PA。 4种 TM联合检测 ,其敏感性明显升高 ,SA+ PA+ CA2 4 2组合敏感性可达 1 0 0 % ,但特异性为 63.46%。 结论 :4种 TM均是诊断大肠癌的有价值的标记物 ,多项指标联合检测提高了检测敏感性 ,可作为人群筛查大肠癌的方法。
OBJECTIVE: To investigate the single and multiple combined detection of sialic acid (SA), polyamine (PA), carcinoembryonic antigen (CEA), and glycoprotein antigen 242 (CA2 4) and other tumor markers (TM) in colorectal cancer. The value in diagnosis. METHODS: Serum levels of SA, PA, CEA, and CA242 were measured in 70 cases of colorectal cancer and 68 healthy adults using HP% M HPLC and HP 1 0 4 A fluorescence detector or enzyme-linked immunosorbent assay. We calculated the sensitivity, specificity, positive predictive rate, negative predictive rate, and total effective rate of single and multiple TM combined tests for colorectal cancer. Results : Four types of TM were significantly higher in patients with colorectal cancer than healthy adults. The single index with the highest diagnostic value of CA2 4 was followed by PA. The combined detection of the four TMs showed a significant increase in sensitivity. The combined sensitivity of SA+PA+CA2 4 2 was as high as 100%, but the specificity was 63.46%. Conclusion : All 4 TMs are valuable markers for the diagnosis of colorectal cancer. Combined detection of multiple indexes improves the detection sensitivity and can be used as a screening method for colorectal cancer.
