目的:调查心内科首诊糖尿病患者胰岛素使用现状及问题,为其使用提供更专业的教育指导。方法:2014年8月至2015年8月,便利选取我院门诊糖尿病患者192例为研究对象,其中心内科首诊的糖尿病患者84例,内分泌科首诊的糖尿病患者108例。使用调查问卷的方式对入选患者进行调查,分析心内科首诊与内分泌科首诊糖尿病患者胰岛素使用现状。结果:按医嘱剂量注射胰岛素比例,内分泌科组76.9%vs心内科组64.3%(P=0.049);选择正确部位注射比例,内分泌科组94.4%vs心内科组84.5%(P=0.023);消毒部位干燥后注射比例,内分泌科组71.3%vs心内科组53.6%(P=0.012);注射前排气比例,内分泌科组73.1%vs心内科组47.6%(P<0.001);注射后停留10s以上拔出比例,内分泌科组96.3%vs心内科组84.5%(P=0.004);注射后从笔上取下针头比例,内分泌科组37.0%vs心内科组22.6%(P=0.032)。结论:心内科首诊的糖尿病患者在注射胰岛素方法上存在严重的不足,迫切需要相关医务人员提供更专业的胰岛素注射宣教。
OBJECTIVE: To investigate the status and problems of insulin use in patients with first-diagnosed diabetes in cardiology and to provide more professional education guidance for their use. Methods: From August 2014 to August 2015, 192 patients with outpatients with diabetes mellitus were selected as the research objects. Among them, 84 were diagnosed with diabetes in the department of internal medicine and 108 were diagnosed as diabetes in the Department of Endocrinology. The questionnaire was used to investigate selected patients and analyze the status of insulin use in the first diagnosis and endocrine department of cardiology in patients with diabetes. Results: The proportion of insulin injected according to doctor’s prescription was 76.9% in endocrinology group and 64.3% in cardiology group (P = 0.049). In the endocrine group, 94.4% vs 84.5% in cardiology group (P = 0.023) After the injection, the rate of injection in the endocrine group was 71.3% vs 53.6% in the cardiology group (P = 0.012). The percentage of excretion before injection was 73.1% in the endocrinology group and 47.6% in the cardiology group (P <0.001) In the endocrine group, 96.3% vs 84.5% in the cardiology group (P = 0.004). After the injection, the proportion of the needles removed from the pen was 37.0% in endocrinology group and 22.6% in cardiology group (P = 0.032). Conclusion: There is a serious deficiency in the method of insulin injection for the first diagnosis of diabetes in cardiology department, and the urgent need for related medical staff to provide more professional insulin injection education.
