【目的】观察尤瑞克林对前循环急性脑梗死患者软脑膜侧支循环的影响。【方法】急性前循环脑梗死患者90例,随机分为观察组和对照组,每组各45例。两组均给予抗血小板、脑保护、调控血压、减轻脑水肿、神经康复等治疗,观察组在上述基础上应用尤瑞克林治疗,分别记录两组在治疗前及治疗14 d后美国国立卫生院神经功能缺损评分(NIHSS),并于治疗14 d后行经颅多普勒超声检查(TCD)检测患侧大脑前动脉平均血流速度(iVACA)与对侧大脑中动脉平均血流速度(cVMCA)的比值(iVACA/cVMCA),及患侧大脑后动脉平均血流速度(iVPCA)与对侧大脑中动脉平均血流速度(cVMCA)的比值(iVPCA/cVMCA),评估软脑膜侧支循环开放情况。【结果】观察组与对照组治疗前NIHSS评分相比较差异无显著性( P >0.05),治疗14 d后两组NHISS评分较治疗前显著降低,且差异有显著性( P <0.05);观察组治疗后NHISS评分显著低于对照组,且差异有显著性( P <0.05)。观察组治疗后总有效率为75.56%,明显高于对照组(53.33% ),且两组相比较差异有显著性( P <0.05)。观察组治疗后侧支循环开放率为73.33%,明显大于对照组(35.56% ),且两组相比较差异有显著性( P <0.05)。【结论】尤瑞克林可有效改善前循环急性脑梗死神经功能缺损情况,有效促进侧支循环开放。“,”Objective] To explore the effects of urinary kallidinogenase on collateral circulation for pa‐tients with acute cerebral infarction of anterior circulation .[Methods] A total of 90 patients with acute cere‐bral infarction of anterior circulation were randomly divided into control ( n =45) and observation ( n =45) groups .Control group received conventional therapy while observation group had additional therapy of urinary kallidinogenase .The scores of National Institutes of Health Stroke Scale (NIHSS) were observed at pre‐thera‐py and Day 14 during follow‐ups .Both groups underwent transcranial Doppler ultrasonographic (TCD) exami‐nation at Day 14 to examine the ratio of mean blood flow velocity (iVACA/cVMCA) between ipsilateral anteri‐or cerebral artery (iVACA) and contralateral middle cerebral artery (cVMCA) ,ratio (iVPCA/cVMCA) of mean blood flow velocity between ipsilateral posterior cerebral artery (iVPCA) and contralateral middle cere‐bral artery (cVMCA) and evaluate the opening of collateral circulation .[Results]After treatment ,the NIHSS score of observation group was versus pre‐treatment of observation group .And the difference of NIHSS score at pre‐and post‐treatment was significant ( P <0 .05) .The clinical efficacy rate was obviously higher than that in observation group (75 .56% vs 53 .33% ,P<0 .05) .And the opening ratio of collateral circulation was obviously higher in observation group than that in control group (73 .33% vs 33 .56% ,P <0 .05) .[Conclu‐sion] Urinary kallidinogenase may improve effectively the symptoms and opening ratio of collateral circulation in patients with acute cerebral infarction of anterior circulation .
