评价儿童先天性漏斗胸的多层螺旋CT表现及其对心脏形态的影响。搜集2011年2月-2011年10月经临床诊断并行CT检查的漏斗胸患儿34例,在PACS形态图像上,测量漏斗胸的Haller指数(HI)、CT凹陷指数(CTDI)、心脏旋转角(CRA)、下肺静脉角(PVA)、心脏压迫指数(CCI)及心脏不对称指数(CAI),评价它们之间的关系。结果表明:34例漏斗胸患儿HI为4.12±1.48、CTDI为2.39±0.70、CRA为(53.52±7.68)°、PVA为(66.51±11.02)°、CCI为2.43±0.95及CAI为1.55±0.56。漏斗胸中重度患儿的CCI及CAI与轻度患儿相比较,差异具有统计学意义,且HI、CTDI与CCI、CAI间分别具有正相关关系;而漏斗胸中重度患儿的CRA及PVA与轻度患儿比较,其差异无统计学意义。不同年龄组间患儿的HI、CTDI、CRA、PVA、CCI及CAI比较,差异均无统计学意义。研究结果表明,多层螺旋CT可准确评价先天性漏斗胸患儿心脏受压时的形态改变。
Evaluation of Congenital Funnel Chest by Multi-slice Spiral CT and Its Effect on Cardiac Morphology. Collecting 34 cases of funnel-chest children diagnosed by clinical diagnosis from October 2011 to October 2011. The Haller index (HI), CT dentition index (CTDI) and heart rotation angle CRA, PVA, CCI and CAI were measured to evaluate the relationship between them. The results showed that HI was 4.12 ± 1.48, CTDI was 2.39 ± 0.70, CRA was (53.52 ± 7.68) °, PVA was (66.51 ± 11.02) °, CCI was 2.43 ± 0.95 and CAI was 1.55 ± 0.56 . There was a significant difference between CCI and CAI in children with severe funnel and moderate degree of mild carotid disease, and there was a positive correlation between HI, CTDI and CCI and CAI Degree of children, the difference was not statistically significant. There was no significant difference in HI, CTDI, CRA, PVA, CCI and CAI between children of different ages. The results show that multi-slice spiral CT can accurately assess congenital funnel chest morphological changes in children under cardiac compression.
