目的探讨狼疮肾炎(LN)活动期与静止期外周血单个核细胞(PBMC)凋亡率、CD3+、CD3+CD4+、CD3+CD8+、CD3+CD4+/Fas、CD3+CD8+Fas、sFas表达的意义。方法56例经肾活检证实的LN,其中活动期38例,静止期18例,均采用酶联免疫吸附试验(ELISA)检测sFas、白细胞介素(IL)-1"和流式细胞仪检测PBMC凋亡率、CD3+、CD3+CD4+、CD3+CD8+、CD3+CD4+Fas、CD3+CD8+Fas指标。结果LN活动期PBMC凋亡率、sFas明显升高;静止期sFas较正常组增高(P<0.01);CD3+细胞比正常组增多(P<0.05)、CD3+CD4+细胞较正常组明显减少(P<0.01)、CD3+CD8+细胞比正常组增多(P<0.01);PBMC凋亡率与sFas呈正相关(r=0.523);活动期、静止期CD3+CD4+、CD3+CD8+、FasL表达极少。结论T细胞亚群的不同凋亡状态在LN的发生发展中具有重大意义,细胞凋亡增加与血清sFas水平相关,sFas可能抑制肾小球系膜细胞凋亡,而且可能是LN进展中的一个因素。
Objective To investigate the significance of the apoptotic rate, the expressions of CD3 +, CD3 + CD4 +, CD3 + CD8 +, CD3 + CD4 + / Fas, CD3 + CD8 + Fas and sFas in active and quiescent peripheral blood mononuclear cells (LNs) . Methods Fifty-six cases of LN confirmed by renal biopsy were enrolled in this study. Among them, 38 cases were active and 18 cases were stationary. ELISA and sFas, IL-1 and flow cytometry PBMC apoptosis rate, CD3 +, CD3 + CD4 +, CD3 + CD8 +, CD3 + CD4 + Fas, CD3 + CD8 + Fas index.Results The apoptotic rate and sFas of PBMC were significantly increased in LN active stage and sFas in quiescent stage were higher than those in normal group (P <0.01); CD3 + cells increased (P <0.05), CD3 + CD4 + cells decreased significantly (P <0.01), CD3 + CD8 + cells increased more than normal group And positively correlated with sFas (r = 0.523). There was very little expression of CD3 + CD4 +, CD3 + CD8 + and FasL in active phase and quiescent phase.Conclusion The different apoptotic state of T cell subsets is of great significance in the development of LN, Increased death associated with serum sFas levels, sFas may inhibit mesangial cell apoptosis, and may be a factor in the progress of LN.
