患者,男,47岁。因发现阴囊皮肤一肿痛性绿豆大结节,自用清凉油、中草药外敷2天后,阴囊皮肤明显肿胀、溃烂,累及阴茎皮肤,伴畏寒、发热,当地求医未愈。半个月后病情进行性加剧。病来排尿无殊。既往无阴囊疾病及糖尿病史。体检:T、P、kPa均正常。阴茎皮肤75％溃烂、坏死,阴囊皮肤95％溃烂、坏死,深达肉膜下,两侧睾丸部分裸露,皮缘不齐,坏死物覆盖创面,恶臭味。两侧腹股沟淋巴结肿大。脓液培养为普通变形杆菌与A型溶血性链球菌生长。坏死组织病理报告为急性阴囊阴茎坏死性筋膜炎。入院后经灭滴灵、氨苄青霉素、丁胺卡 Patient, male, 47 years old.
