Sepsis is one of the leading causes of mortality and health care expenditures in Indian subjects, affecting all age groups.It is characterized by a whole-body inflammatory state that is triggered by an infection.The body may develop this inflammatory response by the immune system to microbes like E.colin the blood, urine or other tissues.The ability to cause infection is by presence of virulence genes that lead the commensal E.coli into potential pathogens.Evolution of virulence may occur by horizontal transfer of genes encoding virulence factors or by functional modification of existing genes.Mutations can bring functional modifications to adapt to the pathogenic life style.The knowledge of phylogenetic grouping and Virulence profile can give insights to source of genetic diversity of the microorganism and this can greatly help in treatment strategies and rational use of antibiotics.Our results showed that 74.5% of the Sepsis isolates were found to be pathogenic strains of Group B2 and D while only 24% of the isolates were found to be commensal strains of group A and B1.We also showed that papG was expressed in 99%, FimH in 92%, TraT in 87% and Aer in 72% of E.coli isolates from Sepsis patients.None of these were found positive in commensal E.coli strains.Combine virulence profile was significantly higher in Sepsis patients as compared to controls, showing the importance of these virulence markers leading to sepsis infection in humans.
