Vascular malformations may be grouped into capillary, venous, and arterial anomalies.Intraosseous vascular malformation (VM) can cause life-threatening bleeding.Various treatment modalities include injection of sclerosant,curettage and packing, embolization, ligation of external carotid artery (ECA) and segmental resection together with immediate reconstruction have been advocated.Pre-surgical embolization followed by surgery is the most common approach.Uncontrolled intra-operative haemorrhage is the most feared complication and post-operative haemorrhagic complication cannot be overlooked in spite of its rarity.This presentation reports a case of mandibular intraosseoues arterial VM managed by the standard combination of superselective arterial embolization and complete curettage of VM via buccal window approach.There was a massive post-operative swelling as an immediate post operative complication.Postoperative computed tomography of head and neck diagnosed as lymphoedema in which mimicking bleeding in surgical treatment of mandibular intraosseous arterial VM.This presentation also illustrates the strategies to minimize intra-operative blood loss and maintenance of intact of inferior alveolar nerve, and especially, highlight the need to recognize post-operative facial lymphoedema, which could mimic a post-operative haemorrhagic complication, to avoid unnecessary surgical exploration.
