目的:观察消水散外敷联合顺铂胸腔内灌注治疗恶性胸腔积液对免疫功能的影响。方法:将60例恶性胸腔积液患者随机分为2组,联合组30例采用消水散外敷联合胸腔内灌注顺铂,单纯组30例采用胸腔内灌注顺铂。比较2组临床疗效、免疫功能指标变化,对比不良反应发生情况。结果:在临床疗效方面,2组比较差异无统计学意义(P>0.05)。单纯组治疗前后CD_3~+、CD_4~+、CD_8~+、CD_4~+/CD_8~+水平差异无统计学意义(P>0.05),但治疗后CD_3~+、CD_4~+、CD_4~+/CD_8~+水平均较治疗前下降,而CD_8~+水平较治疗前升高;联合组治疗前后CD_3~+、CD_4~+、CD_8~+、CD_4~+/CD_8~+水平差异有统计学意义(P<0.05),且治疗后CD_3~+、CD_4~+、CD_4~+/CD_8~+水平均较治疗前升高,CD_8~+水平下降;2组患者治疗后CD_3~+、CD_4~+、CD_8~+、CD_4~+/CD_8~+水平比较差异有统计学意义(P<0.05)。2组患者均有不良反应发生。结论:消水散外敷联合顺铂胸腔内灌注治疗恶性胸腔积液能够控制积液的生长,改善患者的免疫功能,减轻化疗药物不良反应,其机理可能是通过增强患者机体细胞免疫功能,抑制肿瘤细胞活性而发挥抗肿瘤作用。
OBJECTIVE: To observe the effect of Xiaoshonganshu Fu Combined with Cisplatin in the treatment of malignant pleural effusion on the immune function. Methods: Sixty patients with malignant pleural effusion were randomly divided into two groups. Thirty patients in combination group received Xiaoshui Powder combined with thoracic perfusion of cisplatin. Thirty patients in the simple group received thoracic perfusion of cisplatin. The clinical curative effect and immune function index were compared between the two groups to compare the incidence of adverse reactions. Results: There was no significant difference in clinical efficacy between the two groups (P> 0.05). There was no significant difference in the levels of CD_3 ~ +, CD_4 ~ +, CD_8 ~ + and CD_4 ~ + / CD_8 ~ + before and after treatment in simple group (P> 0.05) The level of CD_8 ~ + decreased compared with that before treatment, while the level of CD_8 ~ + increased before treatment. The levels of CD_3 ~ +, CD_4 ~ +, CD_8 ~ + and CD_4 ~ + / CD_8 ~ + in the combined group were significantly different (P <0.05). After treatment, the levels of CD_3 ~ +, CD_4 ~ + and CD_4 ~ + / CD_8 ~ + increased and CD_8 ~ , CD_8 ~ +, CD_4 ~ + / CD_8 ~ + levels were significantly different (P <0.05). Adverse reactions occurred in both groups. Conclusion: The combination of Xiaoshongosan and Cisplatin in the treatment of malignant pleural effusion can control the growth of effusion, improve the immune function and reduce the side effects of chemotherapeutic drugs. The mechanism may be that it can inhibit the tumor by enhancing the cellular immune function of the patient Cell activity and play an antitumor effect.
