目的:通过对82例潘生丁99Tc-MIBI心肌灌注单光子发射计算机断层显像(SPECT)阳性患者的冠状动脉造影(CAG)比较,了解SPECT对具体病变血管检出的特异性和阳性预测值。方法:对1995年6月~1998年5月疑诊冠心病患者行SPECT后,作CAG82例进行对比分析,按照SPECT分区与CAG所显示血管的关系进行同组自身对照。结果:CAG显示冠脉病变:左前降支50支,回旋支32支,右冠脉48支。SPECT缺血分区所代表的冠脉病变:左前降支:59支,回旋支:41支,右冠脉:53支。分析显示左前降支(LAD)、回旋支(LCX)、右冠脉(RCA)被检出的敏感性均为100%,特异性分别为:78%、85%、87%,阳性预测值分别为:85%、78%、91%。结论:潘生丁99Tc-MIBI心肌灌注单光子发射计算机断层显像对冠状动脉粥样硬化性心脏病(冠心病)具体病变血管的定位诊断也能提供可靠的信息。可作为进行介入治疗和判断介入治疗后心肌血运情况的重要依据之一。
OBJECTIVE: To compare the specificity and positive predictive value of SPECT for detection of specific lesion vessels in 82 patients with dipyridamole 99Tc-MIBI myocardial perfusion single photon emission computed tomography (SPECT) coronary angiography (CAG). Methods: From June 1995 to May 1998, patients with suspected coronary heart disease underwent SPECT. CAG 82 patients were compared and analyzed. The same group of self-control was performed according to the relationship between SPECT and blood vessels displayed by CAG. Results: CAG showed coronary artery lesions: 50 left anterior descending coronary artery, 32 circumflex artery and 48 right coronary artery. Coronary artery lesions represented by the SPECT ischemic segment: left anterior descending branch: 59, supranoidal: 41, right coronary artery: 53. The results showed that the sensitivity of detection of LAD, LCX and RCA were 100% and 78%, 85% and 87% respectively, the positive predictive value As: 85%, 78%, 91%. Conclusions: The diagnostic value of dipyridamole 99Tc-MIBI myocardial perfusion single photon emission computed tomography can also provide reliable information on the localization of specific lesions in coronary atherosclerotic heart disease (CHD). It can be used as an important basis for interventional therapy and determination of myocardial blood flow after interventional therapy.
