目的:探讨并得出吉林省健康儿童经体表面积(BSA)调整的冠状动脉的Z值,运用Z值来判断冠状动脉是否异常,从而指导可能影响冠状动脉的疾病的临床诊治。方法:研究收录了2009-05~2012-10吉林大学第一医院儿科超声门诊检查的15岁以下健康儿童,共998例。将其分成6组,分别为≤1岁、1~3岁、3~6岁、6~9岁、9~12岁、12~15岁。使用东芝SSH-880CV仪器分别测量左冠状动脉主干(LMCA)和右冠状动脉(RCA)内径。将测量的内径值转换为经BSA调整的Z值。结果:在不同年龄组的健康儿童之间经BSA调整的LMCA和RCA内径Z值存在明显差异。Z值在低年龄组之间以及高年龄组之间没有统计学意义。结论 :该研究建立了吉林健康儿童冠状动脉Z值的正常区间,运用Z值来判断冠状动脉是否扩张,对于指导川崎病以及其他可能影响冠状动脉的疾病的临床诊治工作有着重要的意义。
OBJECTIVE: To explore and determine the Z value of coronary artery adjusted by body surface area (BSA) in healthy children in Jilin Province and to judge whether the coronary artery is abnormal by using the Z value to guide the clinical diagnosis and treatment of the diseases that may affect the coronary artery. METHODS: The study included 998 healthy children under 15 years of age undergoing pediatric ultrasound examinations at the First Hospital of Jilin University from May 2009 to 2012-10. They were divided into 6 groups, respectively, ≤ 1 year old, 1 to 3 years old, 3 to 6 years old, 6 to 9 years old, 9 to 12 years old, 12 to 15 years old. The diameter of left main coronary artery (LMCA) and right coronary artery (RCA) were measured by Toshiba SSH-880CV instrument. The measured inner diameter value is converted to a BSA adjusted Z value. RESULTS: Significant differences were observed in BSA adjusted LMCA and RCA inner diameter Z values among healthy children of different age groups. The Z value was not statistically significant between the lower age group and the upper age group. Conclusion: This study established the normal range of coronary artery Z value in healthy children in Jilin province. The use of Z value to determine whether coronary artery is dilated is of great significance for the clinical diagnosis and treatment of Kawasaki disease and other diseases that may affect the coronary artery.
