目的探讨紫癜性肾炎(HSPN)患儿血清半胱氨酸白三烯(cysteinyl leukotrienes)与肾脏病理损害之间的关系。方法研究对象为2008年9-12月本院住院临床及肾脏病理确诊为紫癜性肾炎的80例患儿,68例健康体检儿童为对照组。采用酶联免疫吸附法分别检测对照组及实验组儿童血清半胱氨酸白三烯水平,并分析肾脏病理损害程度与血清半胱氨酸白三烯水平之间的关系,探讨24 h尿蛋白排出量与血清半胱氨酸白三烯水平之间的关系。结果①80例紫癜性肾炎患儿急性期血清半胱氨酸白三烯水平(110.218±32.439)ng/ml与68例正常健康体检儿童(35.946±15.606)ng/ml比较,差异有统计学意义(P<0.05);②病理分型II级及病理分型III级患儿的血清半胱氨酸白三烯水平分别为(110.234±24.209)ng/ml、(108.672±23.432)ng/ml,两组比较差异无统计学意义(P>0.05);③病人24 h尿蛋白排出量与血清半胱氨酸白三烯水平之间的关系,按照24 h尿蛋白的排出量分成三组,24 h尿蛋白定量<150 mg组、50 mg/kg>24 h尿蛋白定量>150 mg组、24 h尿蛋白定量>50 mg/kg组,三组的血清半胱氨酸白三烯水平分别为(99.90±16.44)ng/ml、(115.53±28.30)ng/ml、(169.75±18.27)ng/ml,三组间比较,差异有统计学意义(P<0.05)。结论半胱氨酸白三烯与紫癜性肾炎的发生、发展相关,与紫癜性肾炎患儿蛋白尿的排出密切相关。
Objective To investigate the relationship between serum cysteinyl leukotrienes and renal pathological changes in children with purpura nephritis (HSPN). Methods The subjects were 80 hospitalized children with purpura nephritis who were hospitalized in our hospital from January to December in 2008 and 68 children with healthy physical examination as the control group. Conclusions Cysteine leukotriene is associated with the occurrence and development of purpura nephritis and is closely related to the excretion of proteinuria in children with purpura nephritis.
