目的:观察西宁地区老年女性脑梗死(CI)患者急性期血清雌激素水平的变化与血清脂质、CT面积之间的相关性。方法:采用时间分辨荧光免疫法测定急性CI 82例及正常对照组60例的性激素水平;同时利用生化检测仪测定血脂水平;利用64排螺旋CT测定新发病灶的数量和各病灶的最大面积。结果:CI组血清雌二醇(E2)、甘油三酯(TG)、低密度脂蛋白(LDL)水平比正常对照组增高(P<0.01);高密度脂蛋白(HDL)水平较正常对照组降低(P<0.01)。同时在多灶型CI中,血清E2、TG和LDL水平较单灶型CI增高(P<0.01),血清Chol、HDL水平较单灶型CI降低(P<0.01)、梗死面积较单灶型CI小(P<0.05)。结论:老年女性CI患者性激素E2的升高不仅是一种应激反应,而且可能通过影响血脂代谢的机制,缩小梗死的面积。血脂代谢影响CI的梗死面积。
Objective: To observe the relationship between serum estrogen level and serum lipid and CT area in elderly female patients with cerebral infarction (CI) in Xining area. Methods: The levels of sex hormones in acute CI 82 cases and normal control group were determined by time-resolved fluorescence immunoassay. The levels of serum lipids were measured by biochemical analyzer. The number of new lesions and the maximum area of each lesion were determined by 64-slice spiral CT. Results: The levels of serum estradiol (E2), triglyceride (TG) and low density lipoprotein (LDL) in CI group were higher than those in normal control group (P <0.01). The levels of high density lipoprotein Decreased (P <0.01). At the same time, the levels of serum E2, TG and LDL in multi-foci CI were higher than those in single foci (P <0.01), while the levels of serum Chol and HDL were lower than those in single foci (P <0.01) CI was small (P <0.05). CONCLUSION: Elevated sex hormone E2 is not only a stress response in senile female CI patients, but also may reduce infarct size by affecting the mechanism of blood lipid metabolism. Lipid metabolism affects infarct size in CI.
