Objective To study the status quo and problems of community chronic disease management in a city-town junction in Beijing and provide evidence for improving the management of chronic diseases in urban and rural areas. Methods To conduct a personal in-depth interviews with 2 managers and 5 medical workers in a community health service center in Beijing. Results A large number of community-controlled communities in the urban-rural areas in Beijing led to the relative lack of human resources for medical personnel. The speeding-up of urbanization and the establishment of large-scale hospitals reduced the proportion of CHD patients attending community clinics. Economic development The level of cognitive development of health is not consistent with the pace of development, leading to increased difficulty in chronic disease management and health education. Conclusion Policymakers should fully consider the features of the community in urban-rural areas and formulate well-targeted measures.
