目的探讨血清癌胚抗原(CEA)、细胞角蛋白19片段(CYFRA21-1)、神经元特异性烯醇化酶(NSE)、鳞状细胞癌相关抗原(SCCA)、糖类抗原125(CA125)联合检测在肺癌诊断中的价值。方法选取80例肺癌患者、75例肺部良性疾病患者、90例健康体检者,采用电化学发光法检测其血清中CEA、CYFRA21-1、NSE、SCCA、CA125水平。比较3组研究对象血清CEA、CYFRA21-1、NSE、SCCA、CA125水平及不同类型肺癌患者血清CEA、CYFRA21-1、NSE、SCCA、CA125水平,并分析肿瘤标志物单项检测与5项联合检测对肺癌诊断的灵敏度、特异度和准确度。结果肺癌组患者血清CEA、CYFRA21-1、NSE、SCCA、CA125水平均明显高于肺部良性疾病组和对照组,差异均有统计学意义(均P<0.05),小细胞肺癌患者血清中NSE水平较腺癌、鳞癌患者高,差异均有统计学意义(均P<0.05);鳞癌患者血清中CYFRA21-1、SCCA水平较腺癌、小细胞肺癌患者高,差异均有统计学意义(均P<0.05);肿瘤标志物CEA、CYFRA21-1、NSE、SCCA、CA125五项指标联合检测对肺癌诊断的灵敏度、准确度均明显高于单项检测,差异均有统计学意义(均P<0.05)。结论血清肿瘤标志物联合检测可以提高肺癌诊断的灵敏度和准确度。
Objective To investigate the relationship between serum carcinoembryonic antigen (CEA), cytokeratin 19 (CYFRA21-1), neuron specific enolase (NSE), squamous cell carcinoma associated antigen (SCCA), carbohydrate antigen 125 Detection in the diagnosis of lung cancer value. Methods Eighty patients with lung cancer, 75 patients with benign lung disease and 90 healthy subjects were enrolled in this study. Serum levels of CEA, CYFRA21-1, NSE, SCCA and CA125 were detected by electrochemiluminescence. The levels of serum CEA, CYFRA21-1, NSE, SCCA and CA125 in serum and levels of CEA, CYFRA21-1, NSE, SCCA and CA125 in patients with different types of lung cancer were compared among the three groups. The detection of single tumor markers and five combined detection of tumor markers Sensitivity, specificity and accuracy of lung cancer diagnosis. Results The levels of serum CEA, CYFRA21-1, NSE, SCCA and CA125 in patients with lung cancer were significantly higher than those in patients with benign lung disease and controls (all P <0.05). NSE (P <0.05). The serum levels of CYFRA21-1 and SCCA in patients with squamous cell carcinoma were significantly higher than those in patients with adenocarcinoma and small cell lung cancer, the differences were statistically significant (All P <0.05). The sensitivity and accuracy of the combined detection of five markers of CEA, CYFRA21-1, NSE, SCCA and CA125 in lung cancer were significantly higher than those of single test (all P <0.05) <0.05). Conclusion Combined detection of serum tumor markers can improve the sensitivity and accuracy of lung cancer diagnosis.
