目的:评价数字减影全脑血管造影对蛛网膜下腔出血病因诊断的价值。方法:对100 例经腰穿证实的自发性蛛网膜下腔出血患者行数字减影全脑血管造影(其中行 C T 50 例和 M R I40 例)进行病因分析。结果:数字减影全脑血管造影病因诊断阳性率为71.0% (其中动脉瘤28 例,动静脉畸形30 例,动脉狭窄及闭塞病变10 例)、 C T 为34.0% 、 M R I为57.0% 。结论:数字减影全脑血管造影对蛛网膜下腔出血的血管性病变是最直接最可靠的病因检查方法,特别是对动脉瘤与动静脉畸形可明确诊断。但对隐匿性血管畸形 M R I则是较为敏感的和特异性无创伤的检查方法。
Objective: To evaluate the value of digital subtraction whole cerebral angiography in the diagnosis of etiology of subarachnoid hemorrhage. Methods: One hundred patients with idiopathic subarachnoid hemorrhage confirmed by lumbar puncture underwent digital subtraction global cerebral angiography (including 50 cases of C T and 40 cases of M R I). Results: The positive rate of etiological diagnosis of digital subtraction total cerebral angiography was 71.0% (including 28 aneurysms, 30 arteriovenous malformations, 10 arterial stenosis and occlusive lesions), C T was 34.0%, M R I is 57.0%. Conclusion: Digital subtraction whole cerebral angiography is the most direct and reliable method of etiological examination of vascular lesions of subarachnoid hemorrhage, especially for aneurysms and arteriovenous malformations. However, the occult vascular malformation M R I is more sensitive and specific non-invasive examination.
