目的探讨血浆脂联素(APN)、同型半胱氨酸(Hcy)水平及血管内皮功能与2型糖尿病合并高血压的关系,及依那普利叶酸片对APN、同型半胱氨酸的影响。方法选取50例2型糖尿病合并高血压病,50例2型糖尿病无高血压患者,与50例正常对照组,采取酶联免疫吸附法测定各组血清APN水平,采用高效液相色谱(HPLC)法测定Hcy水平,参照Celermajer等方法测定血管舒张功能。对于2型糖尿病合并高血压患者给予口服依那普利叶酸片,观察叶酸片对其APN、Hcy的影响。结果三组中2型糖尿病合并高血压组Hcy水平最高,APN水平最低,依那普利叶酸片治疗后,Hcy水平降低,而APN水平升高。血管内皮功能水平与血清APN呈正相关,与Hcy、BMI、TG、HBA1c呈负相关。结论血清APN水平降低及Hcy水平升高可能是2型糖尿病合并高血压患者血管病变的危险因素,依那普利叶酸片可降低Hcy水平,升高APN水平,进而改善其血管内皮功能。
Objective To investigate the relationship between plasma adiponectin (APN), homocysteine (Hcy), endothelial function and type 2 diabetes complicated with hypertension and the effects of enalapril folate tablets on APN and homocysteine . Methods 50 cases of type 2 diabetes mellitus with hypertension, 50 cases of type 2 diabetes mellitus without hypertension were enrolled in the study. Serum APN levels were measured by enzyme - linked immunosorbent assay (ELISA) in 50 cases of normal controls. High - performance liquid chromatography (HPLC) Method to determine the level of Hcy, with reference to Celermajer and other methods to measure vasodilation. For patients with type 2 diabetes mellitus and hypertension patients were given oral enalapril folate tablets to observe the effect of folic acid tablets on their APN, Hcy. Results In the three groups, the Hcy level was the highest in type 2 diabetes mellitus patients with hypertension and the lowest was APN level. After enalapril folic acid tablets treatment, the Hcy level decreased and the APN level increased. Vascular endothelial function was positively correlated with serum APN and negatively correlated with Hcy, BMI, TG and HBA1c. Conclusions The decrease of serum APN and the increase of Hcy may be the risk factors of vascular disease in type 2 diabetes mellitus patients with hypertension. Enalapril folate tablet can reduce the level of Hcy, increase the level of APN and then improve the endothelial function.
