目的探讨本院处方审方过程中发现的疑问处方存在的问题分析及其对策。方法通过对医院2014年1月-12月期间,审方药师记录的疑问处方进行分析,总结处方审核工作体会。结果所记录的208例疑问处方的疑问类别涵盖用量、诊断与用药相符性、用药频次等各个方面。通过与医师当面或电话沟通的方式,所有审方时发现的疑问处方均得到解决,直接最大限度地降低了疑问处方导致的医疗风险和用药不当。结论分析门诊疑问处方不合理的用药情况,应及时采取相应对策,从而提高门诊用药合理率及处方合格率。
Objective To explore the problems and countermeasures of the questions prescriptions found in the course of the prescription trial of our hospital. Methods Through the hospital from January 2014 to December period, the trial pharmacists record the questions of the prescription analysis, summed up the experience of the prescription audit. Results The 208 suspected cases of suspected problems covered various aspects such as dosage, diagnosis and drug compliance, medication frequency and so on. Through in-person or telephone communication with the physician, all the prescriptions found during the trial were resolved and the direct medical risks and improper medication were minimized directly. Conclusion Analysis of outpatient inquiries prescription unreasonable medication situation, should take appropriate countermeasures in a timely manner, so as to improve the reasonable rate of outpatient medication and prescription pass rate.
