目的:查找住院患者退药的主要原因,优化工作流程,保证用药安全。方法:对医院病区2011年1～6月退药处方进行分类统计,利用帕累托图对本院主要退药原因进行分析。结果:造成退药的前2项(医师调整医嘱和患者出院停止执行医嘱)为主要因素,药品不良反应和患者拒绝用药为次要因素,其余为一般因素。结论:针对造成退药现象的主要原因,应加强医院相关制度的执行力度,提高医师合理用药水平,优化药房摆药流程。
Objective: To find out the main reasons for the withdrawal of inpatients, optimize the workflow and ensure the safety of medication. Methods: The hospital ward from January to June 2011 retreat prescription classification statistics, the use of Pareto chart analysis of the main reasons for the withdrawal of this hospital. Results: The first two items that resulted in the withdrawal (the doctor adjusted the order and the patient discharged to stop executing the order) were the main factors. The adverse drug reactions and the patient refused to take the drug were the secondary factors, and the rest were the general factors. Conclusion: In view of the main reason of withdrawal phenomenon, we should strengthen the implementation of the hospital-related system, improve the rational use of physicians and optimize the drug flow.
