目的探讨全程整体干预对无创呼吸机治疗慢性阻塞性肺疾病(COPD)合并呼吸衰竭患者血气指标的影响。方法选择本院采用无创呼吸机治疗COPD合并呼吸衰竭患者55例,并依据随机数字表法分为治疗组(n=27)和对照组(n=28),治疗组行全程整体干预,对照组行常规护理,两组均连续干预3个月。统计并比较两组患者并发症发生情况及满意度;比较两组患者干预前后血气指标水平变化。结果干预后1个月,两组Pa O2和Sa O2水平均与治疗前比较,呈现显著的上升状态(P<0.01),Pa CO2水平与治疗前相比呈现显著的下降状态(P<0.01),且两组间差异有统计学意义(P<0.05或P<0.01)。p H值护理干预前后及组间比较差异均无统计学意义(P>0.05);对照组并发症发生率显著高于治疗组(P<0.05);治疗组满意度为88.89%,显著高于对照组的64.29%(P<0.05)。结论全程整体干预可有效改善采用无创呼吸机治疗COPD合并呼吸衰竭患者的血气相关指标水平,且并发症发生率低,满意度高,值得临床推广。
Objective To investigate the effect of whole-body intervention on the blood gas indexes of patients with chronic obstructive pulmonary disease (COPD) and respiratory failure without invasive ventilation. Methods Fifty-five patients with COPD and respiratory failure were treated with noninvasive ventilator in our hospital and divided into treatment group (n = 27) and control group (n = 28) according to random number table. Routine nursing, two groups were continuous intervention for 3 months. Statistics and comparisons were made between the two groups of patients with complications and satisfaction; the changes of blood gas levels before and after intervention were compared between the two groups. Results One month after intervention, PaO2 and SaO2 levels in both groups were significantly increased compared with those before treatment (P <0.01), Pa CO2 levels showed a significant decrease compared with those before treatment (P <0.01) , And the difference between the two groups was statistically significant (P <0.05 or P <0.01). There was no significant difference in the value of p H before and after nursing intervention and between groups (P> 0.05). The incidence of complications in the control group was significantly higher than that in the treatment group (P <0.05). The satisfaction of the treatment group was 88.89% Control group, 64.29% (P <0.05). Conclusion The overall intervention can effectively improve the non-invasive ventilator in the treatment of COPD with respiratory failure in patients with blood gas-related indicators, and the low incidence of complications, high satisfaction, worthy of clinical promotion.
