为了探讨巨细胞型胶质母细胞瘤的来源和病理诊断标准,对22例进行临床病理观察和7种试剂的免疫组化检测,并对其中5例做电镜观察。结果显示,22例 GFAP、S-100蛋白和 Vimentin 全部阳性。超微结构的特征是瘤细胞巨大,胞质宽阔,核大畸形,胞质内胶质丝丰富。结论:本瘤来源于星形细胞,属于胶质母细胞瘤的一个亚型。其病理组织学诊断标准为:①具备胶质瘤的Ⅲ级改变;②异型瘤巨细胞数在50%以上;③免疫组化和电镜观察支持为星形细胞源性。 In order to investigate the origin and pathological diagnosis of giant cell glioblastoma, 22 cases were examined by clinicopathological and immunohistochemical methods, and 5 of them were examined by electron microscopy. The results showed that 22 cases of GFAP, S-100 protein and Vimentin all positive. Ultramicrostructure is characterized by huge tumor cells, large cytoplasm, large deformities, and abundant cytoplasmic gliosis. Conclusion: The tumor originated from astrocytes, which is a subtype of glioblastoma. The diagnostic criteria for histopathology were as follows: ① Grade Ⅲ gliomas were changed; ② The number of giant cell in heterotypic tumor was above 50%; ③ The astrocytic origin was supported by immunohistochemistry and electron microscopy.
