目的分析Mammotome乳腺抽吸旋切方法对乳腺病变的诊断和治疗价值。方法对我院2005年7月~2006年2月50例61处乳腺病灶,进行B超引导下Mammotome微创抽吸旋切和活检,评价其诊断和治疗效果。结果本组50例操作无1例失败,乳房皮肤仅留有0.3cm的微小伤口,术后乳腺外形无改变。34例乳腺良性病变全部用Mammotome抽吸旋切切除,经B超证实所有病灶消失。术后随访1~7个月,B超均未见乳腺病灶复发。16例临床Ⅲ期乳腺癌各取得5条组织。34例乳腺病灶抽吸旋切组织送病理检查,其中乳腺纤维腺瘤23例,乳腺腺病5例,乳腺囊肿6例。临床Ⅲ期乳腺癌16例中,乳腺浸润性导管癌14例,小叶癌2例。结论B超引导下Mammotome抽吸旋切系统为乳腺疾病的诊断和治疗提供了一种准确、有效、安全的方法,是乳腺肿物微创治疗的又一进步,具有较高的临床应用价值。
Objective To analyze the diagnostic and therapeutic value of Mammotome breast aspiration and rotation excision for breast lesions. Methods 50 cases of 61 cases of breast lesions in our hospital from July 2005 to February 2006 were subjected to Mammotome minimally invasive aspiration biopsy and biopsy under B-mode ultrasound to evaluate the diagnosis and treatment effect. Results The group of 50 patients failed to operate in one case, leaving only 0.3cm of breast skin minor wounds, postoperative breast shape unchanged. All 34 benign breast lesions were excised by Mammotome aspiration, and all lesions disappeared by ultrasound. Postoperative follow-up 1 to 7 months, B-no recurrence of breast lesions. Sixteen cases of clinical stage Ⅲ breast cancer each obtained five tissues. Twenty-four cases of breast lesions were dissected and sent for pathological examination, including 23 cases of breast fibroadenoma, 5 cases of breast adenosis and 6 cases of breast cyst. Among the 16 cases with clinical stage Ⅲ breast cancer, 14 were invasive breast ductal carcinoma and 2 were lobular carcinoma. Conclusion Mammotome aspiration biomechanical system guided by B-mode ultrasound provides an accurate, effective and safe method for the diagnosis and treatment of breast diseases. It is a further improvement of minimally invasive treatment of breast tumors and has high clinical value.
