目的:探讨胸苷激酶1(thymidine kinase 1,TK1)、癌胚抗原(carcino-embryonic antigen,CEA)及糖链抗原199(carbohydrate antigen 19-9,CA199)检测在结肠癌诊断中的应用价值。方法:选取2015年2月—2016年10月在皖南医学院附属马鞍山市中心医院接受治疗的结肠癌患者为研究对象,同时选取同期接受治疗的结肠息肉患者为对照。观察两组TK1、CEA、CA199表达情况,比较不同临床病理特征结肠癌患者TK1、CEA、CA199的表达情况,并分析影响结肠癌患者TK1、CEA、CA199表达情况的因素。结果:结肠癌患者TK1、CEA、CA199阳性表达率明显高于结肠息肉患者(P<0.001);有淋巴结转移、组织学分型为管腺癌、TNM分期为Ⅲ~Ⅳ期、肿瘤直径≥5 cm的结肠癌患者的TK1、CEA、CA199阳性表达率较高,而不同年龄、性别的结肠癌患者TK1、CEA、CA199阳性表达率无明显差别;将单因素分析有意义的淋巴结转移情况、组织学分型、TNM分期和肿瘤大小作为自变量,将结肠癌患者TK1、CEA、CA199表达情况作为因变量,进行多因素Logistic回归分析,结果淋巴结转移情况和TNM分期是影响结肠癌患者TK1、CEA、CA199表达情况的因素。结论:结肠癌患者的TK1、CEA、CA199阳性表达率较高,且与患者的淋巴结转移情况和TNM分期密切相关。
Objective: To investigate the diagnostic value of thymidine kinase 1 (TK1), carcinoembryonic antigen (CEA) and carbohydrate antigen 19-9 (CA199) in the diagnosis of colon cancer. Methods: From February 2015 to October 2016, patients with colon cancer treated at the Central Hospital of Ma’anshan, Wannan Medical College were selected as the study subjects. At the same time, the patients with colon polyps treated at the same period were selected as the control. The expression of TK1, CEA and CA199 in two groups were observed. The expression of TK1, CEA and CA199 in patients with different clinical and pathological features of colon cancer was analyzed. The factors affecting the expression of TK1, CEA and CA199 in patients with colon cancer were analyzed. Results: The positive rates of TK1, CEA and CA199 in colon cancer patients were significantly higher than those in colon polyps patients (P <0.001). There was lymph node metastasis and histological classification of adenocarcinoma of the ductal adenocarcinoma. The TNM stage was stage Ⅲ ~ Ⅳ, Of colon cancer patients had higher expression rates of TK1, CEA and CA199, but there was no significant difference in the expression rates of TK1, CEA and CA199 between colon cancer patients of different ages and genders. Univariate analysis of the significance of lymph node metastasis, histological score Type, TNM stage and tumor size as independent variables, the expression of TK1, CEA and CA199 in colon cancer patients was taken as the dependent variable, and multivariate Logistic regression analysis was performed. Results Lymph node metastasis and TNM stage were the influencing factors of TK1, CEA, CA199 Factors that express the situation. Conclusion: The positive rate of TK1, CEA and CA199 in colon cancer patients is high, which is closely related to the lymph node metastasis and TNM stage of patients.
