目的探讨实时三维超声心动图技术对无顶冠状静脉窦综合征的诊断价值。方法应用实时三维超声心动图获取10例无顶冠状静脉窦的实时三维和全容积三维数据库,切割图像并观察冠状静脉窦顶壁缺损的形态特征或左上腔静脉回流位置等,其中9例与手术结果对照,1例与CT检查结果对照。结果10例无顶冠状静脉窦中,Ⅰa型2例,Ⅰb型2例,Ⅱb型2例,Ⅲb型4例。实时三维超声可显示Ⅱ型和Ⅲ型无顶冠状静脉窦的顶壁缺损的完整形态及毗邻关系,2例Ⅰa型可显示永存左上腔静脉的回流部位及入口形状,2例Ⅰb型实时三维超声与二维超声均不能提供有价值信息。结论实时三维超声可直视无顶冠状静脉窦综合征Ⅱ型和Ⅲ型的顶壁缺损全貌以及Ⅰa型永存左上腔静脉的回流部位等,可提供二维超声心动图所不能获得的信息。
Objective To explore the value of real-time three-dimensional echocardiography in the diagnosis of non-crowded coronary sinus syndrome. Methods Real-time three-dimensional echocardiography was used to acquire real-time three-dimensional and full-volume three-dimensional database of non-apex coronary sinus. The images were cut and the morphological features of the top of the coronary sinus or the location of the left superior vena cava venous return were observed. The results of control, 1 case and CT examination results. Results In 10 cases of non-apex coronary sinus, there were 2 cases of type Ⅰa, 2 cases of type Ⅰb, 2 cases of type Ⅱb and 4 cases of type Ⅲb. Real-time three-dimensional echocardiography can show the complete morphology and adjacent relationship of type II and type III non-crowded coronary sinus top wall defects. Two types of type Ia can show the reflux site and entrance shape of permanent left superior vena cava. Two type Ib real-time three- And two-dimensional ultrasound can not provide valuable information. Conclusion Real-time three-dimensional echocardiography can directly observe the type Ⅱ and type Ⅲ top-wall defect without capsid sinus syndrome and the type Ⅰa permanent left ventricular superior vena cava reflux site, which can provide two-dimensional echocardiography can not get the information.
