[目的]研究多层螺旋CT灌注(MSCTPI)扫描技术在肾脏原发性神经内分泌肿瘤(NEN)诊断中的临床价值。[方法 ]研究对象选取2014年5月至2016年1月间经病理学检查确诊为肾脏原发性NEN患者42例和肾错构瘤患者44例,同期选取45名健康志愿者作为对照组,采用多层螺旋CT对所有受试者肾皮质进行灌注扫描,分别计算各组的血流(BF)、相对组织血容量(rBV)、峰值时间(TTP)、峰值增强影像(PEI)、表面通透性(PS)、平均通过时间(MTT)和灌注(Perfusion)。[结果]肾脏NEN组的BF、BV值均高于肾错构瘤组和对照组(P<0.05),肾脏NEN组和肾错构瘤组的PS值均明显低于对照组(P<0.05)。肾脏NEN组的rBV、PEI及Perfusion值明显高于肾错构瘤组(P<0.05),但TTP值明显低于肾错构瘤组(P<0.01);肾脏NEN组的rBV和TTP值明显高于对照组(P<0.05),但Perfusion和PEI值明显低于对照组(P<0.05)。[结论]应用多层螺旋CT灌注扫描技术诊断肾脏原发性NEN,能够较好反映与正常组织和良性肿瘤之间的血流动力学指标差异,较好地鉴别肾脏良恶性肿瘤。
[Objective] To investigate the clinical value of multi-slice spiral CT perfusion (MSCTPI) in diagnosis of primary neuroendocrine neoplasm (NEN). [Methods] Forty-two patients with renal primary NEN and 44 patients with renal hamartoma were diagnosed by pathological examination from May 2014 to January 2016. 45 healthy volunteers were selected as the control group, The renal cortex of all subjects was perfusion scans with multi-slice spiral CT, and the blood flow (BF), relative tissue volume (rBV), peak time (TTP), PEI, Permeability (PS), mean transit time (MTT) and perfusion. [Results] The BF and BV values in renal NEN group were significantly higher than those in renal hamartoma group and control group (P <0.05). The PS values in renal NEN group and renal hamartoma group were significantly lower than those in control group (P <0.05) ). The values of rBV, PEI and Perfusion in renal NEN group were significantly higher than those in renal hamartoma group (P <0.05), but the TTP values in renal NEN group were significantly lower than those in renal hamartoma group (P <0.01) (P <0.05), but Perfusion and PEI values were significantly lower than those in the control group (P <0.05). [Conclusion] The diagnosis of primary renal NEN using multi-slice spiral CT perfusion-scan technique can better reflect the difference of hemodynamic parameters between normal tissues and benign tumors and to identify benign and malignant neoplasms.
