目的:分析某地区招收飞行学员心电图检查因室内传导阻滞淘汰情况,为改进招飞体检工作提供参考。方法:收集2004—2016年度鲁豫两省参加空军招飞体检的应届高中毕业生共16 989例,统计每年心电图检查不合格总例数、因心电图检查不合格淘汰例数以及因具体室内传导阻滞(左前分支传导阻滞、左后分支传导阻滞、完全性右束支传导阻滞及双分支传导阻滞)淘汰情况;并探讨在招飞体检中把握室内传导阻滞标准的做法。结果:16 989例中,因心电图检查异常淘汰657例,总淘汰率3.9%。因室内传导阻滞淘汰92例(占心电图检查总淘汰率14.0%),其中,左前分支传导阻滞49例、完全性右束支传导阻滞34例、左后分支传导阻滞6例、双分支传导阻滞3例。结论:招飞体检中,因室内传导阻滞淘汰者以左前分支传导阻滞最为常见,其次为完全性右束支传导阻滞。左前分支传导阻滞的诊断应结合心电图与心向量图进行综合分析,其临床意义需结合其他心电图异常及实验室检查、心脏超声检查等临床资料;对于单纯左前分支传导阻滞的生源,若平板运动试验、心脏彩超、动态心电图等检查综合判断心脏无潜在风险,可以综合评定为合格。
OBJECTIVE: To analyze the electrocardiogram (ECG) examination of recruit pilots in an area due to the elimination of conduction block and provide reference for the improvement of physical examination. Methods: A total of 16 989 high school graduates from Luyu Province participated in the air force maneuver examination from 2004 to 2016 were collected. The total number of failed annual electrocardiogram (ECG) examinations, the number of cases of unqualified electrocardiogram (ECG) Hysteresis (left anterior branch block, left posterior branch block, complete right bundle branch block, and bifurcation block); and to explore the practice of grasping the standard of conduction block in the maneuver test. Results: In 16 989 cases, 657 cases were eliminated due to abnormal electrocardiogram, with a total elimination rate of 3.9%. Ninety-two cases were eliminated due to conduction block (accounting for 14.0% of total electrocardiogram), among which 49 were left anterior branch block, 34 were right bundle branch block, 6 were left posterior branch block, Branch conduction block in 3 cases. CONCLUSIONS: In the physical examination, the left anterior branch block was most common due to the blockade of conduction block, followed by the complete right bundle branch block. The diagnosis of left anterior branch block should be combined with electrocardiogram and cardiac vector diagram for comprehensive analysis, the clinical significance of the need to combine other ECG abnormalities and laboratory tests, such as echocardiography clinical data; for simple left anterior branch block of the students, if the plate Exercise tests, echocardiography, Holter and other tests to determine the integrated heart no potential risk can be comprehensively assessed as qualified.
