目的探讨用B型超声和超声生物显微镜(ultrasoundbiomicroscopy,UBM)联合检查对诊断视网膜脱离的作用。方法对25例视网膜脱离患者的25只眼同时做B型超声和UBM检查,并将检查结果与玻璃体手术观察结果进行比较。结果裂孔性视网膜脱离18只眼,外伤性视网膜脱离7只眼。B型超声图形可分为4种:“C”型、“V”型、“υ”型和“γ”型,它们在本组病例中分别有6、2、4和11只眼。由C型到γ型代表视网膜脱离由单纯后段增殖性玻璃体视网膜病变(proliferativevitreo-retinopathy,PVR)发展到合并前段PVR。UBM图形可分为3种:视网膜浅脱离、环形收缩和前移位,代表了前段PVR由轻到重。结论B型超声和UBM联合检查可准确地诊断视网膜脱离的严重程度,对于前后段PVR的术前诊断、指导手术方案、估计手术预后具有重要的临床意义。
Objective To explore the role of combined ultrasound B ultrasound and ultrasound biomicroscopy (UBM) in the diagnosis of retinal detachment. Methods Twenty - five eyes of 25 patients with retinal detachment underwent simultaneous ultrasound B and UBM. The results of the examination were compared with those of vitreous surgery. Results There were 18 eyes with ruptured retinal detachment and 7 eyes with traumatic retinal detachment. B-mode ultrasound patterns can be divided into four types: “C” type, “V” type, “υ” type and “γ” type, which have 6, 2, 4 and 11 eyes in this group. From type C to type γ represent retinal detachment from simple proliferative vitreoretinopathy (proliferative vitreo-retinopathy, PVR) to the development of anterior PVR. UBM graphics can be divided into three types: retinal detachment, annular contraction and anterior shift, on behalf of the PVR from light to heavy. Conclusion Combined ultrasound B UBM and UBM can accurately diagnose the severity of retinal detachment. It is of important clinical significance for the preoperative and postoperative diagnosis of PVR, guiding the surgical plan and estimating the prognosis of the surgery.
