目的:研究ω-3多不饱和脂肪酸(PUFA)对胃癌病人术后炎性反应和疲劳综合征的影响。方法:42例胃癌病人随机分为对照组(n=23)和试验组(n=19)。对照组病人按低氮、低热量原则,于术后连续5 d行肠外营养(PN)治疗,试验组病人加用ω-3 PUFA。检测两组病人术前和术后第1、3、5天炎症因子IL-1β、IL-2、IL-6、TNF-α、IL-12水平和术前及术后第1、5天肝肾功能,同时记录病人术前和术后第1、3、5、7天的疲劳评分、术后肛门排气时间和并发症的发生情况。结果:术后第3天IL-1β、TNF-α和IL-6浓度最高,两组无显著性差异。术后第5天,试验组IL-1-β、TNF-α和IL-6浓度明显低于对照组(P<0.05)。疲劳评分分值术后第3天最高,试验组病人术后第3、5、7天疲劳评分均低于对照组,两组差异有统计学意义(P<0.05)。两组病人GPT、GOT、肌酐和尿酸术后第5天较第1天均有下降,试验组下降更明显,与对照组比有显著性差异(P<0.05)。结论:ω-3 PUFA能减轻胃癌病人术后的炎性反应和疲劳,促进术后康复。
Objective: To investigate the effect of ω-3 polyunsaturated fatty acid (PUFA) on postoperative inflammatory response and fatigue syndrome in patients with gastric cancer. Methods: Forty-two gastric cancer patients were randomly divided into control group (n = 23) and experimental group (n = 19). Patients in the control group were treated with parenteral nutrition (PN) for 5 consecutive days according to the principle of low nitrogen and low calorie, and patients in the test group were given ω-3 PUFA. The levels of IL-1β, IL-2, IL-6, TNF-α and IL-12 in the two groups were detected before operation and on the 1st, Renal function, and record the patient’s fatigue score on the 1st, 3rd, 5th and 7th days before and after the operation, the time of postoperative anal exhaust and complication. Results: The levels of IL-1β, TNF-α and IL-6 were the highest on the 3rd postoperative day, with no significant difference between the two groups. On the 5th day after operation, the concentrations of IL-1-β, TNF-α and IL-6 in the experimental group were significantly lower than those in the control group (P <0.05). The score of fatigue was the highest on the 3rd day after operation. The fatigue scores of the 3rd, 5th, 7th day after operation in the test group were all lower than those in the control group. There was significant difference between the two groups (P <0.05). The GPT, GOT, creatinine and uric acid in both groups decreased on the 5th day after operation compared with that on the 1st day. The drop in the experimental group was more obvious than that in the control group (P <0.05). Conclusion: Omega-3 PUFA can relieve postoperative inflammatory reaction and fatigue in patients with gastric cancer and promote postoperative recovery.
