目的探讨无创序贯机械通气联合雾化吸入治疗COPD合并II型呼吸衰竭的临床疗效。方法选取慢性阻塞性肺疾病(COPD)合并II型呼吸衰竭患者60例为研究对象,按随机数字法均分为2组(n=30),观察组给予无创序贯通气联合雾化吸入治疗,对照组给予常规有创序贯机械通气治疗,观察2组患者血气分析、血乳酸和呼吸机相关肺炎(VAP)发生情况并进行统计学分析。结果观察组PaO2、PaCO2、SpO2和VAP发生率与对照组相比,差异有统计学意义(P<0.05)。结论无创序贯机械通气联合雾化吸入是治疗COPD合并II型呼吸衰竭的有效方法,可提高SpO2,降低PaCO2和VAP的发生率。
Objective To investigate the clinical effect of noninvasive sequential mechanical ventilation combined with inhalation on COPD with type II respiratory failure. Methods Sixty patients with chronic obstructive pulmonary disease (COPD) and type II respiratory failure were selected as the research objects, divided into two groups (n = 30) according to the random number method. The patients in the observation group received noninvasive sequential ventilation combined with inhalation therapy. The control group was given conventional invasive sequential mechanical ventilation. Blood gas analysis, blood lactic acid and ventilator-associated pneumonia (VAP) were observed in two groups and analyzed statistically. Results The incidences of PaO2, PaCO2, SpO2 and VAP in the observation group were significantly different from those in the control group (P <0.05). Conclusions Noninvasive sequential mechanical ventilation combined with inhalation is an effective treatment for COPD with type II respiratory failure, which can improve SpO2 and reduce the incidence of PaCO2 and VAP.
